A long time ago when I was in nursing school and then when I taught nursing, I remember making care plans. A large portion of the work our student nurses did was to make care plans for their patients. This was an integral part of their clinical experience.

During post clinical conference, which was at the end of each clinical day we went over what happened that day. We also discussed how well the students were able to follow the care plan. How we could improve our care and make patients feel better.

The main focus of our discussion were the patients, their symptoms and how to help them feel better. Each one of our care plan goals started with, "Patients will…..". We emphasized patient education and their wellbeing.

That was a long time ago. I'd think that nursing would have come a long way by this time. I'd think by now nurses would be celebrated as patient advocates and liaison between all the different groups that come in contact with the patients, including the physicians. Unfortunately, this is far from the truth.

Unfortunately, the clock has turned backwards on nursing in a lot of ways. Yes, we have more nurses with advanced degrees. We have nurses in very influential positions in healthcare with a lot of clout. However, it is the clinical bedside nurse who has lost out during this "advancement".

The patients in hospitals are much sicker than they used to be a few decades ago. As a result, their needs are more emergent, more intense and more difficult to take care of. No wonder nurses hop from room to room, patient to patient trying to meet all those needs.

Typically, if a nurse has five patient for an eight hour shift, she has less than an hour and a half per patient to address all their needs, pass out meds, take calls from their family, communicate with the doctor, help the CNA as needed, finish their documentation and oh yes, attend any mandatory inservices their company requires them to take and still be done without any over time.

Sorry… what??? You think I left something out? Oh yeah… the pee break, right? Well they are few and far between for a typical nurse. Does anybody see anything wrong with this picture?

Despite all the chaos and running around, most nurses can't stand to see their patients suffer. So we make sure that we advocate for them. We try our best to educate them on their treatment plan, because its ingrained in us.

We have been taught over and over again that our patients are our primary responsibility and their wellbeing and education is our primary job.

This is what Amanda Trujillo of Arizona did not too long ago. A patient requested information and she provided hospital approved material along with a request for more information from hospice as per patients request. 

This is not unusual for a nurse. In a typical day we do a lot of teaching to our clients, their families, CNAs and anyone else who needs information. The only difference this time perhaps was that the doctor's idea of what the patient needed to know and what the patient actually wanted were different.

This is not unusual scenario. Communication gap happens often. The only way to prevent a communication gap from becoming a disaster, especially in a healthcare setting where life and death decisions are made every minute, is to have check points.

Yes, someone has to go after a doctor to ensure that patient and the family actually did understand what they had been presented with but to also provide more information and listen to their concerns.

At one of the hospitals I worked for, our case managers, who were also nurses, routinely spoke to patients undergoing surgeries and treatment to make sure there was no communication gap. In addition, these case managers communicated with the staff regularly to alert them of issues and help them continue the conversation as needed. This way we all were on the same page and our patients made informed decisions.

In case of Amanda Trujillo and her patient, the patient obviously had concerns. Any nurse, any one who considers themselves a patient advocate, would have provided more information.

It is heartbreaking to see that instead of being praised for her concern for the patient, she was fired from her job and may even lose her license. It is also sad to note that the outburst by the physician at the nursing station was not addressed as unprofessional.

Currently Amanda Trujillo awaits the Arizona board of nursing decision on whether she should be able to keep her license. Her act of advocating for her patients could result in her inability to work as a nurse and earn a livelihood.

This is an outrage! An atrocity that will put nursing in dark ages. This one decision, to take away Amanda Trujillo's license would make the nurses nationwide fear for their license. They would fear that their license could be snatched from them for doing the right thing for the patients if it does not sit right with those in power.

The nursing profession will definitely lose if this were to happen, but the biggest looser will be the patients. There might be people who would set their conscience aside to be able to feed their family. But there will be no patient advocates!

Everyone, nurses and non nurses, patients and future patients, doctors, NPs, PAs etc, are going to be on the receiving side of healthcare. Therefore, we all have a great deal to lose from such an action. 

I urge each one of us to stand up for our rights to receive compassionate and competent healthcare. I urge everyone reading this to contact Arizona Board of Nurses and ask them to clear Amanda Trujillo's license and compensate her for all the pain she has been put through.

      • Collin Gabriel Delete what you will but this will be a PR nightmare for you unless you become a model of how to treat your caregivers versus crippling them financially. You should be celebrating nurses like this. My extended family lives in Arizona and you can rest assured they will hear of this from me and avoid your hospital. I have seen many organizations try to manage situations like this and it only gets worse, especially for the culture within your hospital, so, accolades or no, this is a truly shameful way to treat your employees.

Have any of you been following the story of Amanda Trujillo? Amanda is an AMAZING nurse who is an excellent example as a patient advocate and as a result, lost her job. You can actually read her story here with Vernon Dutton, a fellow nurse advocate. I met Amanda several months ago through social media. As I put up the page on Facebook that represents Nurses On The Edge, Amanda became a fan and we engaged in an awesme discussion about nurses experiencing stress and burnout. She didn’t disclose her entire story at that time to me. We found, however, that we really “got” each other. We experienced a real connection as we shared our hearts and knew we wanted to help other nurses experiencing stress due to corporate pressure, physician’s verbal abuse, and even peer-to-peer bullying.

After our conversation, I began to read about the struggles Amanda experienced as a direct result of being a patient advocate. These words from Amanda caused me to remember a situation many years ago. “Nurses are afraid to speak up when they see unsafe or unethical practice taking place with their patients’ lives”. I remember several years ago as an Intensive Care Nurse, we got a call from surgery. There was a man “coding” in surgery, and you know, there is always so much investigation when someone dies in surgery. So, the physician wanted the patient up in the ICU and out of the surgical suite. It seems the patient was having a “simple” surgical operation and the surgeon “nicked” the portal vein. This particular patient began to bleed uncontrollably and it could not be repaired or stopped. Due to the high blood loss, the patient developed DIC. (Disseminated intravascular coagulation)  This is true. The patient did develop DIC. However the family was told by the surgeon that the patient started bleeding in surgery and developed this disorder. What the physician DID NOT disclose was that he had cut the portal vein so severely that he could not get it repaired in order to stop the bleeding. The patient came to our unit and it wasn’t long before he succumbed to this tragic ending to a simple surgery. I took the patient’s family contact number and for the longest time wanted to make an anonymous phone call and tell them to get an autopsy. Why didn’t I follow through? I regret that now.

To whom it may concern,

In the case of Amanda Trujillo, RN, a profoundly negative and chilling precedent threatens to silence nurses in their efforts to uphold their solemn oaths to protect, educate and advocate for their patients.

Additionally, a patient's Right to Know and Right to Self-Determination are directly in jeopardy.

National Nursing groups, Patients' Rights Advocacy groups, and the media have been put on alert and are watching this case carefully, hoping that the AZ BON makes the right choice to support the education and interventional activities of a nurse, who discovered a severe knowledge deficit in her patient and operated within her scope to rectify it.

It would be a shameful to set a precedent that re-defines our profession and the collaborative health care model that we thought we operated in for the good of our patients.

I respectfully request that you consider dropping the complaint filed against Amanda Trujillo's license and the case against her.

Arizona Nurse Has License Threatened By Doctor After Providing Patient Education

arizonanurseThere are times in my nursing career when I am so very proud of how far we have come in advancing our profession. Unfortunately, there are also times where I am reminded that this is still very much an uphill climb we must make in order to legitimately participate in truly collaborative healthcare. Today is one of those days in which I am reminded we still have battles to win.

The following blog post is related to an email that was originally sent to @EchoHeronAuthor. It was then posted on  Vernon Dutton’s Posterous, Amanda Trujillo case will go before the Arizona State Board of Nursing  on January 24th, 2012.

Her story is one of an archaic medical model in which the doctor’s word is supreme and we are all just nurse maids here to do their bidding. This is an indication that there are many who do not wish to continue to advance toward collaborative healthcare in which we work as a team to provide patients with the best care possible. This is also an example of persons who may not be in medicine for the right reasons.

Success in healthcare is not defined by how many treatments and tests you can perform on a patient. Success is defined by the ability to help a patient make informed decisions on their path in health an life. If that path includes surgery, then so be it. If that path does not, and we have informed the patient of all the options and they are capable of making an informed and rational decision, then we have been successful.

I’m really disgusted that a healthcare organization would bow to the tantrum of one very arrogant and immature physician. If there was one example of a surgeon with a God-Complex, this is one.

I encourage you to read the email and case below and to reach out to Amanda Trujillo (@FYRHONEYBSN on twitter) to show her your support. We need patient advocates like her to take care of us. Do you want your loved ones to have the right of choice in their healthcare stripped away from them by a detached physician?

Do you currently practice as a nurse? If yes in what type of setting? I currently practice as a media television producer and host to educate the public on medical issues.

Your most pressing concerns with the profession and healthcare: Lack of knowledge given in Nursing schools on both theory and practice. Lack of surgical experience and sterile technique. There appears to be a lack of followup. Too many hours spent studying on non medical/nursing subjects. I am also concerned about the changes in health care eliminating time in hospitals etc.

How do you feel the coming nursing shortage (as baby boomer enter the healthcare system, and nurses retire in droves) will affect patient care? The retiring experienced nurses will leave a big gap in the health care system. Both their previous education and experience have saved many lives the dedication and education now appear to lack these attributes.
