When Nurse Make Mistakes (Theresa Brown, RN) - NYTimes.com

This year, a Seattle nurse named Kim Hiatt committed suicide. Ms. Hiatt’s death came nearly seven months after she had given an unintended overdose to an infant heart patient, a medical error that was said to have contributed to the child’s death days later.

Ms. Hiatt had been a nurse for 27 years and had often cared for the 8-month-old girl during the child’s stay in the pediatric intensive care unit of her hospital. She had probably drawn up the right dose of the drug hundreds of times in her career. But once, she made a life-changing error. A baby died, and she was suspended, then fired from a profession she loved. And now she’s dead.

Research and regulatory bodies have long confirmed that poor communication in healthcare is harmful at best and deadly at worst.

In the 2005 study, Silence Kills, VitalSmarts and the AACN found that 84 percent of healthcare professionals observe colleagues take dangerous shortcuts when working with patients and yet less than 10 percent speak up about their concerns.

Since that time, the healthcare community has turned to safety tools and checklists to reduce unintentional slips and errors. And yet, a new study called The Silent Treatment has found that the effectiveness of safety tools is undercut by undiscussables. Every day, healthcare professionals are making calculated decisions to not speak up—even when safety tools alert them to potential harm.

The Silent Treatment reveals that despite the safety interventions taken in the last decade, silence still kills. Safety tools do not compensate for crucial conversations failures in the hospital.

On the Job by Anita Bruzzese: 6 Ways to Handle Criticism at Work

"If something is bothering the boss, you've got to find out what it is," she says. "That's why it's so important that you ask questions, trying to narrow down exactly what is the problem. Don't let the boss talk about past issues because there's nothing you can do about that. Keep him focused on what must be done in the future."

Rosenberg, author of From Rage to Resolution, (iUniverse, $23.95), offers more tips when dealing with criticism at work:

• Control your body language. Don't clench your teeth or fists or cross your arms while listening. Try to sit next to the boss if he's offering feedback such as in a performance review. This sets a more friendly, equitable tone.

• Don't become defensive. Justifying yourself is a waste of time, as is trying to blame someone else.

• Focus on the problem. When the speaker is done, ask questions and then rephrase the issue: "As I understand it, you are concerned about (this problem) and you would like me to (state solution). Is that correct?"

No matter what health-care reform winds up looking like, one thing’s certain: Nurses will play a bigger role than ever, taking over duties from doctors, moving out of hospitals and into the community, looking after frail aging boomers, leading the push to keep costs down and improve outcomes. So this seemed the perfect time for a consult with the people who really run health care in Philadelphia. As one nurse told us, “Doctors only think they’re the quarterbacks.” Here’s what nurses had to say about their work, their patients, life and death, and those little white caps they used to wear.

1 // It’s okay to buzz // Really. Go ahead. Room too cold? Need a pillow? Got a question about your meds? Use the buzzer; that’s why they put it there. “My 82-year-old mother was just in the hospital for a stroke,” says Presbyterian Hospital’s Michael Becker, “and she said, ‘I can only hit the buzzer two times a day.’ I asked, ‘Why is that?’ and she said, ‘Because they’re going to get tired of seeing me.’ I said, “No, no, Mom, it doesn’t work that way.’” If somebody’s pushing the buzzer all the time, nurses know there’s a deeper issue and work to address it.

2 // You don’t have to be afraid // Nurses understand that patients often don’t speak up because they feel vulnerable. But when we don’t voice our thoughts, they have no way of knowing what needs improvement—until the patient-satisfaction survey arrives after the fact. And they hate finding out there was something they could have fixed if they’d known about it. “It’s natural to be fearful,” says Pennsylvania Hospital’s Mary Del Guidice, “because you’re lying in bed with all these faces looking down at you. But don’t be afraid of us.”

Tough Demands on Nurses Adversely Affect Patients

Better working conditions and better staffing of nurses can significantly improve the care of patients with serious conditions, according to the latest nurse labor study by the University of Maryland School of Nursing.

Several troubling trends in patient outcomes surfaced as researchers analyzed survey data from 633 nurses in 71 hospitals in North Carolina and Illinois concerning patient outcomes, says lead investigator Alison Trinkoff, ScD, FAAN.

For example, pneumonia deaths were significantly more likely in hospitals where nurses reported increased psychological demands and more adverse work schedules. Trinkoff says they measured high psychological demands by very fast work, lack of time to complete work, excessive required work, being slowed by delays from other workers, and frequent interruptions. Another trend was that patients were more likely to develop deep vein thrombosis after surgery in hospitals where nurses reported high psychological demands. Staffing also was controlled in the analysis, so that the effects occurred independent of staffing.

A Nurse Confesses:  There is no way to work on a psych ward of a mental hospital and not learn something about life; I have met some of the strangest and most original individuals.  When people find out where I work, and have worked for almost 22 years, their mouths hangs open in awe.  Most of the time the phrase, "I don't know how you do it" is mentioned, as they shake their heads.

 

I confess there are things about working in a mental health institution that I do not like, and there are times when I have to bite my tongue and keep my lips glued together because I become so agitated.  I thought I would list for you my dislikes and explain later what I have learned.  Deep breath…here I go.

 

I dislike when someone comes into the hospital just so they can get a check (aka crazy check) when they are clearly healthy but truly too damn lazy to work.

 

I dislike when someone is purely and simply mean spirited and uses their diagnosis of being mentally ill as an excuse to cling to.

The first column I ever wrote for The New York Times, called “Perhaps Death Is Proud, More Reason to Savor Life,” generated a firestorm of attention. Literary agents sent me e-mails, my piece hit The New York Times “most e-mailed” list, and within three days I’d been offered a book contract with a major publisher.

 

The column described a sudden and grisly cardiac arrest where a patient with lung cancer exsanguinated. I felt happy and lucky when The New York Times accepted it. I thought the piece would come out and my friends would read it, and that’s where the endeavor would end. Instead, from that one piece, I ended up becoming a professional writer about nursing. My book, Critical Care: A New Nurse Faces Death, Life, and Everything in Between (HarperCollins), was published in June 2010 and comes out in paperback in April 2011. In addition, I am a regular contributor to The New York Times’ Well blog.

 

Whenever I talk about my two careers, people often ask the same questions. I have answered some of those recurring questions here, in part, because that is what interests other nurses, but also because those questions get at the heart of how I combine these two very different jobs.

Is Nursing Making You Sick, Fat and Old? Annette Tersigni “The Yoga Nurse”, RN

As nurses, you are certainly aware that stress is one of the major medical problems of our times.  Studies show that nurses are the single sickest group of workers in the workforce.  Ouch!  This is appalling. When I worked nights in the PCU, I noticed that there were many more obese nurses working nights than days.  What have you observed?   In my experience, I have noticed that without exception, obese nurses apply to work nights. They want to work nights. These dedicated, long term night shift nurses also are on multiple meds and often discuss how many meds they are on, compare them, etc.  What strikes me is that they speak about it in such a way as if it’s the most natural thing in the world.  Please understand, I am writing this out of compassion for my fellow nurses, not as a critique.  I am telling the truth, and yes, sometimes the truth hurts.  We all know by now that working nights increases your risk for disease.

A nurse is the classic poster child for, “Is this job making me sick?”
