Eplerenone, How Does it Work, DrugDigest.org:"Eplerenone works by blocking aldosterone, a chemical that causes water and salt retention that can increase blood pressure. By blocking aldosterone, eplerenone causes the kidneys to get rid of unneeded water and salt from the body in the urine."

Eplerenone, Mechanism of Action, MosbysDrugConsult.com:"Eplerenone binds to the mineralocorticoid receptor and blocks the binding of aldosterone, a component of the renin-angiotensin-aldosterone-system (RAAS). Aldosterone synthesis, which occurs primarily in the adrenal gland, is modulated by multiple factors, including angiotensin II and non-RAAS mediators such as adrenocorticotropic hormone (ACTH) and potassium. Aldosterone binds to mineralocorticoid receptors in both epithelial (e.g., kidney) and nonepithelial (e.g., heart, blood vessels, and brain) tissues and increases blood pressure through induction of sodium reabsorption and possibly other mechanisms.

Eplerenone has been shown to produce sustained increases in plasma renin and serum aldosterone, consistent with inhibition of the negative regulatory feedback of aldosterone on renin secretion. The resulting increased plasma renin activity and aldosterone circulating levels do not overcome the effect of eplerenone on blood pressure.

Eplerenone has relative selectivity in binding to recombinant human mineralocorticoid receptors compared to its binding to recombinant human glucocorticoid, progesterone and androgen receptors."

Using eplerenone (Inspra), Dear Patient, NursingDrugGuide.com:"Your health care provider has prescribed the antihypertensive drug eplerenone to treat your hypertension. This drug works by blocking the effects of aldosterone, a hormone normally found in your body. The results of this block may be an increase in urination and an increase in serum potassium levels. The name of this drug is Inspra."

Eplerenone, Indications and Dosage, RxLIst.com:"INSPRA is indicated for the treatment of hyper-tension. INSPRA may be used alone or in combination with other antihypertensive agents.

INSPRA may be used alone or in combination with other antihypertensive agents. The recommended starting dose of INSPRA is 50 mg administered once daily. The full therapeutic effect of INSPRA is apparent within 4 weeks. For patients with an inadequate blood pressure response to 50mg once daily the dosage of INSPRA should be increased to 50 mg twice daily. Higher dosages of INSPRA are not recommended either because they have no greater effect on blood pressure than 100 mg or because they are associated with an increased risk of hyperkalemia. (See CLINICAL STUDIES .) No adjustment of the starting dose is recommended for the elderly or for patients with mild-to-moderate hepatic impairment. For patients receiving weak CYP3A4 inhibitors, such as erythro-mycin, saquinavir, verapamil, and fluconazole the starting dose should be reduced to 25 mg once daily (see CONTRAINDICATIONS)."
