Listening to nurses is key to being a good doctor, KevinMD, #nurseup #nursefriendly
I’m a doctor. We get all the glory. And credit. And guess what? We only deserve part of it.
I started out in medicine in the mid-80′s, volunteering at an ER. And the biggest shock to me was learning how much of what happens in a hospital is nurse territory. Doctors will see you anywhere from 5 to 30 minutes a day, depending on how sick you are. And the rest is the nurses.
They’re the ones making sure you get your pills and checking that your vital signs aren’t dropping. They make sure you don’t fall down and break something. If you start vomiting, doctors will run out of the room and the nurses will rush in. They change your wound dressings and start your IV line. They’ll bring you a warm blanket. And clean disgusting things off you. Even if you’re drunk. Or delirious. Or mean. And through all of this they try be friendly and positive. Even though you aren’t their only sick patient.
I respect nurses. I learned early on that they’re key to being a good doctor. You piss off the nursing staff, and you’ll have a miserable career at that hospital. Respect and treat them well, and you’ll never regret it. They’re as important to being a good doctor as your medical degree. Maybe more.
If you come out of medical school with a chip on your shoulder against nurses, you better lose it fast. Because they will make or break your training, and often know more than you do. Be nice and they’ll teach you. A good neurology nurse is often a better inpatient neurologist than some doctors I’ve met.

For more than a decade, patient safety leaders have urged medicine to shift from an approach that shames and blames individual doctors and nurses for medical errors to a "culture of safety" where open discussion and reporting about adverse events, mistakes, disruptive behavior and unsafe conditions are prized rather than punished.
This less-punitive model of medical-error prevention, inspired by the aviation industry's safety record since the 1980s, is a key element of the Joint Commission requirements hospitals must follow to get paid by Medicare. And a growing body of evidence is showing that higher safety culture scores are correlated with better clinical outcomes and lower rates of hospital-acquired conditions.
Yet data released in February by the Agency for Healthcare Research and Quality show that most physicians, nurses, pharmacists and other health professionals working in hospitals believe their organizations are still more interested in punishing missteps and enforcing hierarchy than in encouraging open communication and using adverse-event reports to learn what's gone wrong.
Half of the nearly 600,000 staffers surveyed at more than 1,110 hospitals nationwide said they believe their mistakes are held against them, and 54% said that when an adverse event is reported, "it feels like the person is being written up, not the problem."
Nearly two-thirds said they worry that mistakes are being held in their personnel file. A little less than half of respondents said they "feel free to question the decisions or actions of those with more authority

Seclusion and Restraint causes 80% of Nurse Burnout #nurseup #nursefriendly
A crisis can be defined as a moment in time when an individual in your charge loses rational, and at times even physical, control over his or her own behavior. This can be very challenging and anxiety producing for those responsible for intervening. Due to the chaotic, unpredictable nature of a crisis, it is vital that staff stay calm and proceed with a plan. These crisis moments do not sprout into being without roots; there are almost always warning signs that let you know an individual’s behavior is escalating.When the CODE D buzzer sounds – everyone in the building pauses to hear the instructions and location of the crisis. All available staff start running to the area to assist, as you turn the corner you go into a high alertness to see what is going on and what needs to be done to help.
Staff arrive in groups, usually way more people than would ever be needed to handle a crisis. The client or persons who are having the crisis begin to change levels of behavior by just seeing people arrive. They do not always calm down and regain control just because they are outnumbered – some see it as more of a challenge.
