Aging Motorcycle Riders

A new study shows that motorcycle owners in the United States are getting older. The researchers looked at injured motorcyclists aged 17 to 89 years in the National Trauma Databank invovled in crashes from 1996 to 2005. Age trends and injury patterns were assessed over time.

Injury Severity Score (ISS), length of stay (LOS), intensive care unit (ICU) use, comorbidities (other illnesses), complications, death, injury patterns, helmet use, and alcohol use were compared for subjects 40 and older versus those younger than 40 years old.

There were 61,689 subjects included. Over the study period, the mean age increased from 33.9 to 39.1 years, and the proportion of subjects 40 years of age or older increased from 27.9 to 48.3 per cent. ISS, LOS, ICU LOS, and mortality were higher in the 40 years of age or older group. The rates of admission to the ICU (32.3 vs. 27.3%), pre-existing illnesses (20 vs. 9.7%), and complications (7.6 vs. 5.5%) were all higher in the 40 years of age and older group.

Motorcycle riders are getting older and more seriously injured. Motorcycle accidents can cause devastating injuries- shearing of skin, fractures, head injuries, massive bleeding. I have worked on several that resulted in fatalities, and a few that involved defects in the bike that resulted in the crash. All of the cases involved young men under the age of 30.

The average age of the injured motorcyclist is increasing. Older riders’ injuries appear more serious, and their hospital course is more likely to be challenged by illnesses and complications contributing to poorer outcomes. Motorcycle safety education and training initiatives should be expanded to specifically target older motorcyclists.

Nurses and Anti-Vaccination, Posted on June 4, 2012:"As a health care provider I consider it my professional duty to stay current on recommended vaccinations, including the annual flu vaccine. I have the option of receiving it at my work site (limited times/places so not very convenient) or at my doctor’s office, or—as I usually opt for—at my local Safeway pharmacy. My annual flu shot comes between shopping for groceries, is immediately covered in full by my health insurance, and is administered by a very friendly and knowledgeable pharmacist within about five minutes of registering for it. (I fully realize all of these lack of barriers/facilitators do not exist for everyone). I have read the evidence on risks and benefits of the annual flu vaccine and am convinced on the side of getting the shot. I listen respectfully to my nurse colleagues who opt out of the flu vaccine, but I am dismayed by how many opt out for anything other than religious or valid medical reasons. I haven’t tried to convince them to change their minds because I never saw that as my job. But if any of them happen to be reading this, I hope they will at least reconsider their decision."

Organ Donation Informed Consent, Is A Single Parent’s Sufficient? #nurseup #nursefriendly #healthcare

Summary: Organ donors are in high demand. Frequently intended recipients can wait a lifetime for the critical matching organ. In this case, two nurses obtained a consentfrom a child's mother. When the father later expressed his disagreement, the child's corneas had been harvested and it was too late.

The patientwas an 11-year-old child that had died soon after an Asthma attack.

"By condition (excluding pregnancy), the five leading causes of hospitalization among children are: bronchitis/asthma, digestive disease, pneumonia and respiratory infection, otitis media and mental disorders."1

The parents were approached for the purpose of donating the child's corneas under the Uniform Anatomical Gift Act.

"What is a Corneal Transplant?

The cornea is a dime-sized clear tissue covering the front of the eye. Light rays pass through the cornea and then through the lens. The lens forms an image on the retina in the back of the eye where the optic nerve is located. Sight is controlled by the optic nerve, the only nerve of vision. This nerve activates the retina to pick up the image in view."2

The mother alone was present at this time when it was first discussed.

"The child's mother maintains that she told the nurseit "did not matter" to her."3

"The Uniform Anatomical Gift Act (UAGA) of 1968 provided for the first time that an individual could donate his or her organs at death to another for medical purposes. It was adopted in all 50 states and the District of Columbia, and was a major step toward solving the organ donation problem. But new medical technologies have now made many transplant operations commonplace, and a shortage of donor organs remains."4

With this, the harvesting center was contacted and the patient's information left. A consent form was faxed over to the hospital. Two nursesthen spoke to the mother by telephone and signed off on the form that consent had been obtained.

Individual state laws may vary in procedure for who can consentto an anatomical gift. In Alaska for instance:

"(a) A competent person who is 18 or more years of age may make a gift to take effect upon death of all or a part of the person's body for a purpose specified in AS 13.50.020.

(b) When persons in prior classes are not available at the time of death, and in the absence of actual notice of contrary indications by the decedent or actual notice of opposition by a member of the same or a prior class, any of the following persons, in order of priority listed, may give all or a part of the decedent's body for a purpose specified in AS 13.50.020:

(1) the spouse; (2) an adult son or daughter; (3) either parent;
(4) an adult brother or sister; (5) a guardian of the decedent at the time of death; (6) any other person authorized or under obligation to dispose of the body. (c) The persons authorized by (b) of this section may make the gift after or immediately before death."5

The agent for the organ center came and harvested the child's corneas and returned to the center. He had looked over the consentform obtained, initialed it to verify it had been obtained by phone and was satisfied.

"When the cornea, a thin tissue that covers the front of the eye, becomes cloudy or damaged due to disease, injury or hereditary conditions, the result is vision loss or even blindness.

To clear this window, the damaged cornea is removed surgically and replaced with healthy, transplanted eye tissue-a donated cornea. This transplant operation is successful in more than 90 percent of cases in the US. After a successful transplant, patients have renewed vision or see for the first time."6

Each individual facility must generate protocols on organ procurement and obtaining informed consent:

"Each hospitalin the state shall develop procedures for identifying potential donors of gifts, requesting gifts, notifying and coordinating with eye banks, tissue banks, and organ procurement agencies, and assisting in the procurement, removal, storage, and transportation of gifts."7

The child's father would arrive on the scene soon after. When approached and informed of the harvesting, he was perturbed. He would not sign the consent after the fact and chose to take legal action against the hospitaland the donor center.

In court, summary judgement was entered for the defense:

The parent's appealed.

Questions to be answered:

1. Under the Uniform Anatomical Gift Act (UAGA), is the consentof a parent sufficient to proceed with harvesting of a child's organs.

2. Were the nursesnegligent in their explanation of the procedure or in not waiting till the father was available?

The applicable laws stated that as long as a consent from a legally "responsible" and informed party is obtained, no liability or negligence can be assigned. It was clear from the testimony of the nurses that they spoke to the mother and informedher of what was to happen.

In obtaining organs, time is typically of the essence. The longer an organ or tissues remains in a body, the less likely it will be useful for the purposes of transplantation.

There was no evidence that the mother was either "coerced" or "rushed" into making a decision. There was no documentationthat she "wished to speak to her husband" before making the decision. If more time had been needed or another family member needed to be consulted, the mother could have clearly stated this.

On the part of the nurses, a single adult guardian consentonly was needed and obtained.

When the agent of the eye bank looked over the informed consent, he as satisfied that it was legitimate. In harvesting the child's corneas, he acted in good faith and had no reason to believe a proper consenthad not been obtained.

The court dismissed the plaintiff's argument that the "good faith" actions of the hospital or eyebank were "subject to interpretation." The courtwas satisfied that under the conditions, the actions of the employees were reasonable and within the boundaries of existing law.

This case does point out the need for judgement and clear documentation when a consent for organ donation is obtained. The nurses were wise to obtain not one but two witness signatures on the consentform. The agent of the eye bank was wise to make sure the notation was made of a "telephone" consent.

Regardless of these precautions, the nurses, hospital and eyebank were still sued. Had this lawsuit been initiated after either of the employees involved had left, the hospitalmay or may not have covered or defended them.

Advocating, Leading, Caring: The Journey to Capitol Hill, Amanda Trujillo, MSN, RN #Nurseup #AmandaTrujillo #Nursefriendly

There are many hurdles I have yet to overcome in my own state toward the goal of protecting my colleagues, their ability to protect and teach patients, and patients’ rights at the end of life. As far back as I can remember I have read stories in People Magazine and Oprah Magazine. I’ve seen stories on the internet, national news programs, and have watched movies based on real life events on the Lifetime Channel. (Don’t laugh) Regular people like you and me accomplish the impossible  every day. They overcome the hecklers, the naysayers, the seemingly impossible hurdles.  THEY MAKE IT TO CAPITOL HILL. THEY GET HEARD, THEY MAKE CHANGE TO HELP OTHERS. I used to think that change for our profession was primarily–and solely dependent on the leaders within our profession and within the various nursing organizations—namely, the American Nurses Association–of which I am a member.  I was convinced it was the responsibility of the upper echelons within our profession to do all the work and make sure the troops in the trenches at the bedside were heard and protected. It never occurred to me that the bedside nurse could make an impact, that we had the privilege or ability to go to Capitol Hill and be heard by our country’s leaders. I don’t know why I thought you had to have some kind of professional license or “in” to be able to speak.  I never wanted to be in a position like this, or a situation like this, or even be involved in politics. Like a plane drops objects to the ground from 35,000 feet in the air now and then, life has a way of plopping things down in front of you, without warning,  out of the blue. There is no rewind button, no fast forward button, no slow motion replay function…no “system restore point.”

What I do know for sure is that a path has been determined for me and regardless of what occurs in “other situations” I am going to Capitol Hill to testify about what our profession is enduring right now in this moment, the risks we take, the struggle we face to choose between ourselves and our patients, the damage we sustain to our physical and emotional well being, and the difficulties we experience trying to practice to the full extent of our education and licensure. I want legislation in all 50 states that protects bedside nurses from retaliation while performing the duties we took an oath to do and are mandated to do via the Nurse Code of Ethics. Right now, only twenty three states have laws in place to protect nurses from retaliation when advocating in good faith for the safety and well being of their patients. Twenty three states isn’t enough. In the past year several cases of retaliation have occurred and I wonder how many more have taken place that we don’t know about. How many other nurses have lost their livelihoods, sustained PTSD, and left the profession because of reporting patient safety concerns. The point is, it is happening more often and in many states at once—it has to stop, NOW. This isn’t just an issue about our practice and the future of nursing– its about establishing a safe nest for our young so they can develop and grow in a healthy way, its about the bigger issue of protecting people’s lives, their well being, their basic human rights, and their right to the best healthcare. We went to school to be the protectors, the teachers, the healers, the innovators, and the leaders that are supposed to help shape healthcare policy and contribute to healthcare reform.

Most of us went to school ( and stayed in school ) in pursuit of something more than a “job.” We went to school because we were called by “life” to be nurses and want nothing more than to spend our lives caring for people whether it be at the sunrise or sunset of life. Its that love, that pride, that passion, that belief in something greater that pushes some of us to the very edge overlooking an awfully deep and rugged canyon. It’s that innate desire to do the right thing that causes some us to remain focused on making it to the other side of the canyon by simply trying our wings–no matter the odds. This is my nurse journey, but at the same time I share it with  many of you out there who have written to me and those of you who have remained silent and forever changed by your experiences. I have set a goal and I intend to achieve it but would like to ask all you nurses out there–I would welcome you–to write me. 1. Have you ever been retaliated against in any way when trying to advocate for your patient or speaking up about workplace safety issues

2. How did it make you feel

3. Did it impact your desire to remain in nursing

4. Did the experience(s) affect your health either emotionally or physically

5. Why you initially wanted to become a nurse

I have learned enough this past year to know that there are scores of nurses dealing with the emotional and physical scars they have sustained by trying to do their jobs as nurses, –protecting patients. I have received enough letters to know that many nurses are transitioning out of the profession and surrendering their licenses. I have read enough facebook messages, emails, tweets, and gotten enough phone calls from nurses across the country to know that nurses are the silent walking wounded–ashamed–living with devastating mental illnesses such as major anxiety depressive disorder and PTSD as a direct result of our profession, unsure of what to do with their lives or who they are…..Most importantly I have read stories of lives cut short, whether it be a patient’s life, or nurses who attempted suicide multiple times out of guilt for not speaking up to prevent a patient’s death, or embattled in a nurse bullying situation or a nurse retaliation situation. It should not hurt to be a nurse, at least not in the ways we are hurting today. Nursing should not make nurses feel like they have to choose between their patients and themselves, or that they need to take their own lives, or that they are worthless used baggage– hopeless after the unnecessary loss of a career and life calling. Family members of nurses have written to me tell of watching their loved ones go in and out of psychiatric treatment centers after experiencing “survivors guilt”(because they feared speaking up),  nurse retaliation or persistent nurse bullying in the workplace. They describe the gradual loss of a once vibrant  partner, a mother, a sister–that their loved ones were never the same. What we do every day as nurses is phenomenal. Its the stuff miracles are made of. We are the backbone and the spirit of healing in this country– ironically its us who need the support and healing the most.

I want to take people’s stories to Capitol Hill with me. I want to collect as many as I can, I want to read as many as I can to the members of Congress so that the voices of so many are finally heard and respected. This is a way you all can “advocate, lead, and care”  by just telling your own personal story–without risk, without fear. This is a way you can make it to Capitol Hill and be accounted for. If you wish to go along for the ride and go to Capitol Hill with me then by all means let me know that too. But Im not waiting anymore for people to advocate for me or for my colleagues or for a profession I love dearly and the work of caring that is so much a part of who I am. Its my responsibility as a nurse, and a citizen to do the work necessary to make the change that is needed. Im tired of seeing position statements, reading articles, and hearing about new initiatives–none of which address the true life experiences of the bedside nurses. There is no better time to “do” rather than “talk” than now. Every second we wait, every day that goes by patient’s lives are on the line, a nurse is forced to choose between herself and her patient, a nurse loses her livelihood…or worse his/her life. When Nurses Week 2013 rolls around I want to know that I’ve truly accomplished something that will help our profession retain more nurses, raise awareness about the need to heal and support each other, and strengthen the voice of Nurses.
