1,000 Shades of Nursing: Why are we “really” celebrating Nurses Week? May 9, 2012 By Amanda Trujillo, MSN, RN, @nurseinterupted:”There was an interesting tweet chat taking place this evening with regards to Nurses Week and how everyone celebrated their week or reflected on their profession. Several great view points were offered up. My own perception of Nurses Week is that it is a metaphor for every way our profession has stagnated, failed to progress, remaining fractured and at odds with itself about where it should be and how it should get there. Hell, we’re still trying to bridge the gap between academic nursing and clinical nursing. It’s not difficult to empathize with some of the views raised this evening: The disdain with the recycled food offerings, the traditional ice cream social that makes some of us seethe inside, the hospital logo stamped on nursing “swag,” the lack of any gifts of appreciation, the “lumping in” of Nurse Week with “Hospital Week,” or whether we even need the proverbial “pat on the head” as one nurse blogger put it. Im conflicted: As a profession—HOW are we leading, and HOW are we advocating?

This Nurse’s Week, I thought it important to share my insightful interview on self-care safety with Dr. Mary Foley, former President of the American Nursing Association (ANA). She shares her expertise with us on Worker Safety and Safe In Common. 

What has been your experience with the general “culture” or perception by nurses and other healthcare professionals regarding needlestick safety?

In my experience, healthcare workers are focused on patient care. They are aware of needlestick risks, but they keep their worries about occupational injury in the background, so they can focus on giving the best care they can. That presents some danger to healthcare workers. Doctors, nurses, aids, lab technicians, housekeeping, and maintenance personnel are all at risk when sharps devices and delivery systems are not designed for maximum protection.

Medical Ethics and Me, Thursday, January 26, 2012 #NurseUp:”Amanda Trujillo is a transplant nurse and patient advocate in Phoenix, Arizona. That is, she was.
Trujillo was fired in Spring 2011, lost her license, and cannot at present hold another nursing position in Arizona. And it seems that she was fired for being a transplant nurse advocating for a dying patient. Calling it ironic doesn’t do it justice.
Trujillo’s advocacy was either in defiance of, or at least without regard for, a transplant physician who had other plans for the patient that involved surgery and certain realities of post-surgical care about which the patient was not aware. Trujillo admits that the physician’s concerns were not hers, and she clearly has strong feelings about how unaddressed revenue pressures can compromise institutional ethics and practices. What also is clear is that the best interests of the patient were Trujillo’s priority. Which is pretty much what I want in a nurse.”

Corruption of the medical literature is impossible to prevent by Bradley Evans, MD #nurseup #healthcare #nursefriendly #s4pm:”This article argues that there can be no certainty that information is unbiased, fair and accurate, because the individual physician will not be able to detect two common deceptions.

The first is to omit key facts deliberately. The doctor won’t know what was left out. You can’t know what you aren’t told. One example of that is Bombardier et al.’s rofecoxib (Vioxx) study published in the NEJM. Merck and its ghostwriters left out the fact that some patients had had heart attacks. When the editors looked at the CD containing the article, they found it was in Microsoft Word, which contains prior versions, as well as the finished article. Prior versions showed that the sentence mentioning the heart attacks had been deleted. Now, pharmaceutical companies and their ghostwriters are smart enough to send articles without including prior versions, so that discovery of this deception, which was embarrassing and costly for Merck, will not occur again.

While the first deception assumes the person transmitting information is corrupt, the second assumes that the data are corrupt. The person transmitting the information may be above reproach. How can this happen? It can occur several ways: (1) Negative studies can be discarded, with only the positive ones published; (2) Studies can be done with comparison to placebo, not to best known treatment; (3) Studies can be ghostwritten by companies paid by the study sponsor; (4) Editors can be biased by ad revenue from drug companies, the main revenue source for most journals; and (5) An editor may be a paid investigator for drug company. This is probably not an exhaustive list.

”I discovered that they had a very big misunderstanding about what they were about to participate in,” Trujillo told CBS 5 News.
Trujillo, a licensed nurse in Arizona since 2006, said she advised the patient of possible complications.
The patient decided to delay the surgery and reconsider his or her options.
Trujillo said she also ordered a case management consult for the patient to be educated about hospice care.
”The doctor, ultimately, is the focal point that directs care for patients,” said Banner spokesman Bill Byron, who said company policy prevents nurses from ordering a case management consult.”
