Hospice, Palliative Care Nurses, Nurses for a Healthier Tomorrow/STTI:"The hospice movement has evolved in the United States over the past 25 years. The focus of hospice care is on comprehensive physical, psychosocial, emotional, and spiritual care to terminally ill persons and their families. Hospice providers promote quality of life by protecting patients from burdensome interventions and providing care at home, whenever possibly, instead of the hospital. Hospice nurses provide care primarily under the guidelines of the Medicare Benefit Act of 1983, a federal program that allows patients to die in their homes with their families and friends at their side."

The organization is committed to improving end of life care and expanding access to hospice care with the goal of profoundly enhancing quality of life for people dying in America and their loved ones. Considered to be the model for quality, compassionate care at the end of life, hospice care involves a team-oriented approach of expert medical care, pain management, and emotional and spiritual support expressly tailored to the patient's wishes. Jacob "Jack" Kevorkian Dies; Death With Dignity Proponent Remembered

Jacob "Jack" Kevorkian, otherwise known as "Doctor Death" fell victim Friday to a blood clot that released from his leg and lodged in his heart, assumed to be complications of kidney problems for which he was admitted to a hospital for in May of this year. He was 83 years of age.

Kevorkian is most known for serving eight years of a 10-to-25-year prison sentence for second-degree murder starting in 1999. This resulted from a videotape he had made on September 17, 1998 which depicted the voluntary euthanasia of Thomas Youk, with the doctor's assistance by administering lethal injection.

Though his methods were controversial, the attention he prompted to the issue of "end of life" choices and "right to die" will only benefit patients as government agencies and lawmakers were forced to address the issues.

Patients with do-not-resuscitate orders fare worse | Reuters

A new study could help people with a do-not-resuscitate order make better decisions about what surgeries they are willing to undergo.

About seven in 10 Americans die with such an order, which instructs healthcare workers not to use life-prolonging treatment if a patient's heart or breathing stops.

But other situations that aren't necessarily covered in the do-not-resuscitate, or DNR, orders may also be worth considering, researchers say.

The new report, out Monday in the Archives of Surgery, shows DNR patients have a higher chance of dying following surgery, although not with all kinds of procedures.

I don’t want to say goodbye
Don’t want you to see me cry
But in the end, that sweet by and by
We will meet again.

You remind me, Lee, of me
A bound-up soul, set free
Life struggles overcome
A chance to bask in the sun.

All too short, this life seems
But in not-so-distant dreams
We know this fading world
Is but a path to the King.

Our King, He came, like me
In humanness, to be
Then hung upon that tree
To set us free.

He knew hot tears and cool springs
The love that friendship brings
Now he hears angels sing
So will we.

You and I,
We’ve known sweet-smelling children, rough-bearded men
Giggles with girlfriends, aloneness, amends,
Fear beyond reason, and joy in its season
Love is the reason. Amen.

Palliative care can lead to higher quality of live and longer survival

by John Schumann, MD

Every once in awhile amidst the weekly deluge of medical articles comes a show-stopper that has the potential to change the way we practice.

Rarer yet is the article that not only changes how we practice, but calls into question why we do what we do.

This recent article from the New England Journal of Medicine is just such an article. Before you bolt, let me simplify it for you.

Here’s the quick take.

The authors enrolled 150 patients with advanced lung cancer and split them up between usual care, which included chemotherapy, and an intervention arm, in which they received the same standard of care treatment plus early enrollment in palliative care.

This American Death: The Movie

How has the pursuit of a good or natural death been altered by advances in medical technology which now can almost always extend life, if only for a few minutes, hours or days? Who does not question their Do Not Resuscitate order with death staring them in the face? How can doctors be expected to manage death when they receive so little end-of-life training? And where can a person go to die in peace, if they have no family and do not want to endure hospital procedures?

This American Death explores the complicated world of death and dying in contemporary America, examining the cultural and systemic issues which conspire against Americans experiencing a so-called good death. Despite the consensus that exists among Americans about how they wish to die - surrounded by loved ones, pain free and relatively unaided by technology - why do so many still die in hospitals, in pain, supported by machines? The film looks at why, when a good death is seemingly achievable, few actually experience one?

Anticipatory Grief | Alzheimer's, Dementia & the Stages of Grieving

anticipatory_grief_and_alzheimers
What is anticipatory grief?

Anticipatory grief is the emotional pain of losing a loved one, felt in advance of the person's death. It's a common phenomenon among those who care for the terminally ill.

It may be experienced at any time by any one connected to someone with Alzheimer's, but it's especially common at the middle and late stages of the disease.

Why Alzheimer's caregivers are vulnerable to anticipatory grief

Good-byes are always painful, and Alzheimer's is the ultimate "long good-bye." Anticipatory grief is rampant, and perhaps inevitable, among Alzheimer's caregivers because of the slow, progressive, and incurable nature of the disease. University of Indianapolis researchers asked more than 400 caregivers the open-ended question, "What would you say is the biggest barrier you have faced as a caregiver?" The majority -- more than 80 percent -- referred to the loss of the person they used to know.

The friends and family of someone with dementia experience two difficult psychological states at once:

  • Anticipatory grief, coping with the very real feelings of loss for someone who is still alive
  • Ambiguous loss, interacting with someone who's not fully present socially or psychologically
