Your physician has you come in to his office and run on a treadmill while you're hooked up to an EKG. For the next 8 to 12 minutes, he'll evaluate your heart rate, breathing, and blood pressure as the intensity of the workout increases. When the stress test is over, he'll tell you whether you have coronary artery disease.

Here's news that might make your heart skip a beat: For women, there's a 35% chance the test results will be wrong.

Most often, the test reveals false positives, meaning healthy women are told they have heart disease. Less frequently but obviously far more dangerous is when the test fails to detect clogged arteries that could, in fact, cause a heart attack. Fewer men are misdiagnosed.

Obama budget targets brand name medicines, Health.Yahoo.net

Big pharmaceutical companies could face increased competition from generic drugmakers under two proposals put forth by the Obama administration on Monday despite earlier savings extracted from drugmakers as part of last year's healthcare law.

President Barack Obama, as part of his 2012 budget proposal, called for cutting the number of years drugmakers could exclusively market brand-name biologic drugs to 7 years from 12.

He also set his sights on ending controversial "pay-for-delay" deals that affect traditional, chemical drugs by giving the U.S. Federal Trade Commission power to block them. Under such pacts, brand-name and generic drugmakers settle patent challenges with payoffs that delay lower-cost rivals from reaching the market.

The proposals face a tough challenge of getting through the divided Congress, but could alter the landscape for consumers' access to cheaper medicines.

They quickly drew industry protests.

Watts, H. David : Starting the I.V.

This poem explores the act of inserting an intravenous line (I.V.) into a patient just prior to induction of anesthesia or sedation. The physician-narrator is initially full of bravado, stating "I am good at this" and "I'm the best". The physicality of the act is detailed: the vein "lies stretched and succulent" and the needle "waits / like a mosquito attached / by its sucker." By the end of the second stanza, however, when the I.V. has been successfully inserted, the significance of this seemingly simple medical intervention is stated: "I am suddenly aware / I am connected to his brain.

Heart Advice from a Cardiologist - Prevention.com

I'm 62 and, thankfully, have never been hospitalized, nor can I remember calling in sick. I still work 12-hour days, walk the golf course, hit the heavy bag, and, not too long ago, even played ice hockey. Other than some minor colds, flus, and heartburn, I have been very healthy, and I take no regular medications. How I've managed this isn't a secret. My philosophy is in my books. But for this special issue of Prevention, I want to summarize it for you--what I eat, how I exercise and beat stress, even my own advice that I struggle with. Here's my personal Rx--feel free to make it your own:


Eat three squares and a snack: I start most days with a three-egg omelet (one yolk and whites) and some green tea with a scoop of Benefiber (a natural fiber supplement). For lunch, I usually order baked salmon and grilled veggies from a nearby restaurant. I never eat fast food. And for dinner, my wife, Sari, typically makes fish or chicken with veggies. We eat little starch at home. If I get hungry during the day or when I'm traveling, I'll have a mozzarella stick, fruit, wasabi-coated soy nuts, other nuts, or a high-fiber bar.

Obesity Alone Raises Risk of Fatal Heart Attack, Study Finds

Obese men face a dramatically higher risk of dying from a heart attack, regardless of whether or not they have other known risk factors for cardiovascular disease, a new study reveals.

The finding stems from an analysis involving roughly 6,000 middle-aged men, and it suggests that there is something about carrying around excess weight that contributes to heart disease independent of risk factors such as high blood pressure, diabetes, high cholesterol and arterial disease.

What exactly that something is, however, remains unclear, although the researchers suggest that the chronic inflammation that typically accompanies significant weight gain might be the driving force behind the increased risk.

"Obese, middle-aged men have a 60 percent increased risk of dying from a heart attack than non-obese middle-aged men, even after we cancel out any of the effects of cholesterol, blood pressure and other cardiovascular risk factors," noted study author Jennifer Logue, a clinical lecturer of metabolic medicine with the British Heart Foundation's Cardiovascular Research Centre at the University of Glasgow, in Scotland. "This means [that] obesity itself may be causing fatal heart attacks through a factor that we have not yet identified."

Logue and her colleagues report their observations in the Feb. 15 online issue of Heart.

To explore the subject, the authors spent nearly 15 years tracking 6,082 male patients who were diagnosed with high cholesterol but had no history of either heart disease or diabetes.

Over the study period, the research team noted 214 heart disease fatalities, along with another 1,027 heart attacks and/or strokes that did not result in death.

Patient Stories May Improve Health, NYTimes.com

The only reservation that he mentioned was the same one all the other patients had — he feared that death would come before the perfect organ.

But during one visit just before he finally got the transplant, he confessed that he had been grappling with another concern, one so overwhelming he had even considered withdrawing from the waiting list. He worried that he would not be strong enough mentally and physically to survive a transplant.

In desperation, he told me, he had contacted several patients who had already undergone a transplant. “That’s what made me believe I’d be O.K.,” he said. “You doctors have answered all of my questions, but what I really needed was to hear the stories about transplant from people like me.”

Patients and doctors have long understood the power of telling and listening to personal narratives. Whether among patients in peer support groups or between doctors and patients in the exam room or even between doctors during consultations, stories are an essential part of how we communicate, interpret experiences and incorporate new information into our lives.

Despite the ubiquitousness of storytelling in medicine, research on its effects in the clinical setting has remained relatively thin. While important, a vast majority of studies have been anecdotal , offering up neither data nor statistics but rather — you guessed it — stories to back up the authors’ claims.

Now The Annals of Internal Medicine has published the results of a provocative new trial examining the effects of storytelling on patients with high blood pressure. And it appears that at least for one group of patients, listening to personal narratives helped control high blood pressure as effectively as the addition of more medications.

Monitoring the blood pressure of nearly 300 African-American patients who lived in urban areas and had known hypertension, the researchers at three-month intervals gave half the patients videos of similar patients telling stories about their own experiences. The rest of the patients received videos of more generic and impersonal health announcements on topics like dealing with stress. While all the patients who received the storytelling DVD had better blood pressure control on average, those who started out with uncontrolled hypertension were able to achieve and maintain a drop as significant as it had been for patients in previous trials testing drug regimens.

“Telling and listening to stories is the way we make sense of our lives,” said Dr. Thomas K. Houston, lead author of the study and a researcher at the University of Massachusetts Medical School in Worcester and the Veterans Affairs medical center in Bedford, Mass. “That natural tendency may have the potential to alter behavior and improve health.”

Experts in this emerging field of narrative communication say that storytelling effectively counteracts the initial denial that can arise when a patient learns of a new diagnosis or is asked to change deeply ingrained behaviors. Patients may react to this news by thinking, “This is not directly related to me,” or “My experience is different.” Stories help break down that denial by engaging the listener, often through some degree of identification with the storyteller or one of the characters.

“The magic of stories lies in the relatedness they foster,” Dr. Houston said. “Marketers have known this for a long time, which is why you see so many stories in advertisements.”

In health care, storytelling may have its greatest impact on patients who distrust the medical system or who have difficulty understanding or acting on health information because they may find personal narratives easier to digest. Stories may also help those patients who struggle with more “silent” chronic diseases, like diabetes or high blood pressure. In these cases, stories can help patients realize the importance of addressing a disease that has few obvious or immediate symptoms. “These types of patients and diseases may be a particular ‘sweet spot’ for storytelling,” Dr. Houston noted.

This particular benefit from stories comes as welcome news not only for patients but also for doctors, who are increasingly reimbursed based on patient outcomes. “There’s only so much the doctor can do, so providers are looking for innovative ways to help their patients,” Dr. Houston said. While more research still needs to be done, the possibilities for integrating storytelling into clinical practice are numerous. In one possible situation, which is not all that dissimilar from popular dating sites, doctors and patients would be able to access Web sites that would match patients to videos of similar patients recounting their own experiences with the same disease.

Dr. Houston is currently involved in several more studies that will examine the broader use of storytelling in patient care and delineate ways in which it can best be integrated. Nonetheless, he remains certain of one thing: Sharing narratives can be a powerful tool for doctors and patients.

“Storytelling is human,” Dr. Houston said. “We learn through stories, and we use them to make sense of our lives. It’s a natural extension to think that we could use stories to improve our health.”

5 Ways to Reduce Nursing Turnover in Year One, healthleadersmedia.com

Turnover among first-year nurses remains a huge cost driver and source of frustration for hospital managers. It's hard enough to find these skilled clinicians, and even more annoying that they quit, just when they should be settling into their new careers. That leaves harried HR staff to start the process anew and with no more assurances of retaining the next new recruit.

Beyond the hard-and-fast cost of finding and on-boarding replacements or hiring temps, first-year nurse turnover impacts patient care. It also signals larger workforce management issues, most notably a failure to effectively engage employees and sell them on the mission.  

There are theories about why first-year nurses quit. Perhaps some of these new nurses weren't trained well in school, a cold fact that comes home quickly in the life-and-death hospital setting. Some new nurses probably have unrealistic expectations that collide with on-the-job realities. Some nurses get better offers elsewhere for their high-in-demand skills. Maybe, just maybe, your hospital is not a good place to work. Whatever the reason, the problem persists.
