    It All Comes Down to Posture My Friend, Putmebacktogether.com

    So we’ve all seen it… the elderly male who presents with such an exaggerated forward-flexed posture it looks as though he will fall over if not for the cane propping up his torso with every step, or the middle aged woman who’s “hunch” or kyphosis is accentuated further by her significant loss of lordosis, stiff pelvis, and short hamstrings.  Certainly age related changes and conditions occur, we become stiff, our bones become brittle and our discs become more rigid and narrow.  Aside from this, far too many cases of preventable postural deformities occur and unfortunately, once a person is in the later stages of life they are almost impossible to correct.

    Of course posture related deformities are no longer exclusive to the older population, it is now more prevalent than ever in our youth.  Children and teenagers are receiving increased medical care for posture related conditions; chronic back pain is at the top of the list.  I see far too many young men and women with significant slouched postures.  Imagine how many hours are spent in front of the TV or computer today vs. even 10 years ago.  The last statistic I read stated up to 6-7 hours per day!  Paired with a lack of awareness and decreased activity, poor posture is an epidemic in our society and one that is not going away any time soon.  

    Therefore, it is increasingly important for those of us in the healthcare field, especially in the Physical Therapy profession, to educate our patients and their families on the importance of using proper posture with daily activities while increasing physical activity.  In addition, we need to increase awareness of the deformity that can occur later in life and the painful conditions that may occur right now.   I always tell patients the best thing they can do to improve their posture is to simply to, “be aware of it.”  Aware of it in the car, aware of it on the bus, aware of it at their desks, aware if it as they are standing, walking, squatting, sitting, bending, talking, exercising, cleaning, playing, eating and most of all when they are doing any of these things for a prolonged period of time.  Let’s teach our youth to be proactive; a little effort today will go a long way and will likely prevent illness and injury in the future.  Does it really surprise you that you’re Mothers and Grandmothers were right?

    Respecting the world outside the doors of critical care

    I’m an ‘ICU’ nurse. I’ve been in critical care pretty much my whole career in one way, shape, or form. Every nurse has their forte’, and mine is the world of critical care.

    Maybe it’s because I’m an adrenal junkie?

    Some would say it’s because I’m a man (the good ole’ male-nurse stereotype).

    Or maybe it’s my need for ‘control’? I don’t know. What I do know is when I step outside those doors of critical care my heart starts to race just a lil’. Each time I am outside my comfort zone I gain more respect for those nurses who do it every day.

    The other day I worked on the telemetry floor. Working in a smaller rural hospital it’s part of the job. When it’s the census or staffing, when you work in the ICU it’s part of your job to ‘float’ to other departments when the need arises.

    It was my turn the other day.

    I must tell ya, I couldn’t remember the last time I worked ‘outside’ of ICU? Don’t’ get me wrong, it’s neither good nor bad, it’s just been the luck of the draw. Going from a 2-3 critical care patient assignment to a 5-8 telemetry assignment doesn’t sound like much, but it requires a whole new way of thinking and prioritizing.

    I really don’t know how they do it??

    Can relate to this working agency and going from unit to unit, place to place.

    10 most expensive errors in healthcare settings, KevinMD.com

    Medical errors have been in the news lately.  An Ontario provincial review probing unnecessary surgeries at a Windsor hospital found significant concerns with the work of a pathologist involved in a mistaken mastectomy case.

    In the US, avoidable medical errors added $19.5 billion to the nation’s healthcare bill in 2008, according to a claims-based study conducted for the Society of Actuaries (SOA). The report lists the 10 most expensive errors in healthcare settings.

    Here are the 10 most expensive types of medical errors:

      1. Pressure ulcers–374,964 errors, $10,288 per error and $3.858 billion total.

    2. Postoperative infections–252,695 errors, $14,548 per error, $3.676 billion total.
    3. Mechanical complication of a device, implant or graft–60,380 errors, $18,771 per error, $1.133 billion total.
    4. Postlaminectomy syndrome–113,823 errors, $9,863 per error, $1.123 billion total.
    5. Hemorrhage complicating a procedure–78,216 errors, $12,272 per error, $960 million total.
    6. Infection following infusion, injection, transfusion, vaccination–8,855 errors, $78,083 per error, $691 million total.
    7. Pneumothorax–25,559 errors, $24,132 per error, $617 million total.
    8. Infection due to central venous catheter–7,062 errors, $83,365 per error, $589 million total.
    9. Other complicaitons of internal (biological) (synthetic) prosthetic device, implant and graft–26,783 errors, $17,233 per error and $462 million total.
    10. Ventral hernia without mention of obstruction or gangrene–53,810 errors, $8,178 per error and $440 million total.

    They most expensive errors on a per-error basis are:

      1. Postoperative shock–$93,682.

    2. Infection due to central venous catheter–$83,365.
    3. Infection following infusion, injection, transfusion or vaccination–$78,083.
    4. Gastrostomy complications, infection–$66,765.
    5. Complications of transplanted organ–$66,658.
    6. Infection and inflammatory reaction due to internal prosthetic device, implant and graft–$62,265.
    7. Tracheostomy complications–$56,479.
    8. Gastrostomy complications, mechanical–$55,219.
    9. Infusion or transfusion reaction–$51,686.
    10. Gastrostomy complications–$49,115.

    The report highlights the need for hospitals to reduce errors.  It also serves as a wake-up call to patients and family members.  Now that you know where the errors occur, you know a bit more what questions to ask when you or a loved one is hospitalized.

    Doctors, nurses often contribute to patients' weight problems - FierceHealthcare

    Although some doctors and nurses seems to think stigma and shame can help motivate patients to lose weight, the opposite seems to be true, according to a doctor's commentary published today in the Los Angeles Times.

    "People who are exposed to stigmatizing situations are more likely to engage in unhealthy eating behaviors and less likely to be physically active," said Rebecca Puhl, director of research at the Rudd Center for Food Policy and Obesity at Yale University, who was interviewed for the piece.

    Indeed, most women in one study coped with stigma over their weight by eating more food or refusing to diet.

    What's more, humiliating interactions may make overweight patients unwilling to seek out medical care, which means their other medical problems likely will go untreated, as well. Puhl says that healthcare providers need to adjust their expectations, pointing out that losing weight isn't just about having patients go on diets. An inability to diet down to a healthy weight isn't due to just lack of motivation, according to Puhl.

    She also calls on healthcare providers to recognize that even relatively small changes in weight count as progress toward better health. Most people can't lose more than 10 percent of their body weight and keep the weight off over time, she says.

    Dr. Valerie Ulene, the commentary's author and a preventive medicine specialist whose siblings tortured her when she was an overweight child, says that patients who are overweight deserve to be treated compassionately and effectively. "It's not just the right thing to do, it's the best approach for successful treatment," she writes.

    Are you similar to many other people and a little too attached to your cellphone? Does it seem only appropriate that when your cellphone rings you should answer it the vast majority of the time regardless of who you are with or what you are doing? Are there Twitter or Facebook alerts popping up every three seconds? Is there really anything wrong with multitasking and taking the occasional call during dinner, or checking e-mail while you are in a meeting?

    Believe it or not, your cellphone is possibly causing you more harm than good. The majority of us probably need to take a look and change how we manage our cellphone use. Honestly, how many people can you see right now? How many are nursing their iPhone like it's a newborn?

    Don't get me wrong, I am just as attached to my cellphone as anyone. My Droid literally changed my life; e-mail and texts are an essential part of my day and I honestly don't remember how I existed without instant access to the internet.

    But, have you noticed how cellphones have a tendency to just take over and constantly demand a significant part of your attention? You can go through life on autopilot jumping from one e-mail to the next, responding to voice mail messages, and not really ever paying enough attention to the experiences happening right in front of you. It is not that you are totally oblivious to what is going on, but let's face it, if you are continually being disrupted by your cellphone, you are just not nearly as engaged as you could be.

    Cellphones are literally masters of distraction and they can take your attention away from just about anything instantly and consistently.

    The Negatives to Being "On Demand"
    The ability to instantly connect with anyone has its advantages, but it also has its costs if not managed properly. Don't underestimate the damage caused by allowing your cellphone to constantly require you to multitask.

    Research has proven that workers distracted by email and phone calls suffer from many issues, even including a fall in IQ during the period of distraction. "Those who are constantly breaking away from tasks to react to email or text messages suffer similar effects on the mind equivalent to losing a night's sleep." The same study also found multitasking has a negative physical effect, prompting the release of stress hormones and adrenaline.

    And, the usual justification that multitasking allows you to accomplish more also doesn't really stand up to scrutiny. Another study confirmed that people who multitask actually end up being less efficient.

    Stop Being Used By Your Cellphone
    Stop allowing your cellphone to hold your attention and mind hostage. It is time to reclaim your attention span.

    Now, this doesn't mean that every once in a while you can't make an exception and take a call or check an e-mail while you are doing something else. The problem is when switching back and forth from one task to cell phone management is your standard operating procedure. And, if we notice this is one of our bad habits, we need to start work on changing it.

    5 Tips to Help Free You From Being Held Hostage By Your Cellphone:

    1. Turn off the ringer from time to time.
      As scary as that may sound, there are just sometimes you shouldn't be interrupted. Reserve blocks of times to devote your complete attention to things you need to get done, your kids, your spouse, your driving! Turn your ringer off during holidays and other family or alone time so you can really engage and enjoy those experiences.

  • Turn off your notification light.
    Make a leap of faith and realize everything will not explode just because you don't instantly know when an e-mail or text message comes in. Yes, this may take some getting used to, but, it really is liberating and allows you to decide the best time to read e-mails and texts instead of always checking your phone every time you notice the notification light flashing.

  • Use your cell phone; don't let it use you.
    It's great that you can do just about everything on your cellphone, but when you pick it up to use it, decide what you are going to do, use it, and get out. If you are going to return e-mails, then don't end up surfing around the internet. Have a plan every time you pick up your cell phone and stick to it.

  • Screen your calls and e-mails.
    Prioritize paying attention to who you are with or what you are currently doing. Take a look at who is trying to contact you and decide if you really need to respond right away. No one really knows whether or not you are available so you can decide to not answer your cell phone unless it really is necessary.

  • Reserve blocks of time to check your messages and respond.
    Take periods of time to check and return all your messages. Perhaps you have a block of time you take every morning, afternoon, and evening. This way your messages don't accumulate and you can be fairly confident you are not going to miss anything really time sensitive because you are checking your messages regularly. If you are a heavy user, you may need to have more check-ins throughout the day. Figure out what works best for you, but the key is to not consistently and continually check messages throughout the day.
  • Manage Your Cellphone and Everything Will Improve
    If you are interested in really getting the most out of all your experiences, increasing your productivity and being less stressed and overwhelmed, start paying attention to how you are using your cellphone. You will be amazed at the positive impact that managing your cellphone in the right way will have on everything you do.

    PT Perspective: Heart Disease in Women - Overshadowed and Understated|putmebacktogether.com

    It’s good to be informed. As PTs, I’d add that it’s our responsibility to inform ourselves then to pass on the information to patients. I’ve come across many women, patients, and co-workers who recognize the dangers of breast cancer and the necessity of research to cure it.

    Clearly, breast cancer is serious business and the stats are grim. The lifetime prevalence: 1 in 8; number of deaths per year: 40,000.
    But consider this. Lung cancer claims 70,000 women a year which is more than breast, ovarian and uterine cancers combined. Of those diagnosed with lung cancer, only 1 in 8 survives.  The public consciousness seems misinformed on this.

    Heart disease is another poorly understood killer. Every year, heart disease claims the lives of 500,000 women. That’s 1000% more deaths than breast cancer. People seem to think breast cancer is the biggest monster out there gobbling up women’s lives while the real boogey man disease lives in virtual anonymity.

    Why does this happen? Why would such a big problem get such short shrift? Part of the problem is the stereotype that only type A men get heart attacks.  The fact is heart disease doesn’t dabble in gender politics. Whether you’re from Venus or Mars, the odds are pretty even.
    If you’re a woman, your sirens should be ringing (blaring stridently; in fact), particularly if you have a family history, like to wash down your corn-fed Angus cuts with a keg of beer, and exercise by walking from the couch to the kitchen during commercial breaks. If this describes you or your patients, beware, but don’t despair.

    Giving up leads to paralysis and paralysis doesn’t help. What does help is information (keeping in mind that the step after information is informed action).  Unlike breast cancer which is largely genetic and difficult to prevent (if such as word even applies), heart disease responds to the following life-saving changes:

    • Smoking cessation
    • A well-balanced diet, low in saturated and trans fat
    • Regular exercise
    • Weight loss
    • Stress reduction
    • Social support
    • Decreased alcohol consumption 
    Those who campaign against breast cancer do a wonderful job of raising awareness about a terrible disease. We should applaud these efforts and replicate them with heart disease, spreading the word the way rumors spread in the girl’s middle school bathrooms across America.  First whispered conspiratorially, the message gets passed on via three very simple words: “Pass it on!”
