Soup may be the ultimate comfort food. It’s warm, it’s nourishing, and you don’t even have to chew it. No wonder Mom made it whenever you were sad, cold, or just plain under the weather.

But here’s a dirty little secret: A lot of soups out there aren’t comfort food at all. They’re discomfort food. That’s because they can make for a lot of discomfort every time you step on the scale, test your blood pressure, or try to squeeze into last year’s swimsuit. Nurses Long Work Hours, Scheduling Can Increase Patient Mortality

A new study has found that patient deaths from pneumonia and acute myocardial infarction were significantly more likely in hospitals where nurses reported schedules with long work hours. The finding was just one of several revelations from a study of nurses' work schedules, patient outcomes, and staffing led by University of Maryland School of Nursing researchers in collaboration with researchers at the Johns Hopkins University School of Medicine.

The study is the latest in ongoing research on nurse scheduling and staffing funded by the National Council of State Boards of Nursing. In the current study, Alison Trinkoff, ScD, MPH, RN, FAAN, professor at the School, and co-authors Meg Johantgen, PhD, RN; Carla Storr, PhD, MPH, RN; Yulan Liang, PhD; Ayse Gurses, PhD;and Kihye Han, MD, RN shifted their focus from the effects on nurses in previous studies to patient well-being.

The team linked patient outcome and staffing information from 71 acute-care hospitals in two representative states (Illinois and North Carolina) with the survey responses of 633 randomly selected nurses who worked in these hospitals. Their findings are published in "Nurses' Work Schedule Characteristics, Nurse Staffing, and Patient Mortality," in the January/February issue of the journal Nursing Research. Most U.S. hospitals use 12-hour nursing shifts exclusively, as opposed to eight-hour shifts, a trend begun during nursing shortages nationwide in the 1980s. "Although many nurses like these schedules because of the compressed nature of the workweek, the long schedule?as well as shift work in general?lead to sleep deprivation," says Trinkoff.

Tough Demands on Nurses Adversely Affect Patients

Better working conditions and better staffing of nurses can significantly improve the care of patients with serious conditions, according to the latest nurse labor study by the University of Maryland School of Nursing.

Several troubling trends in patient outcomes surfaced as researchers analyzed survey data from 633 nurses in 71 hospitals in North Carolina and Illinois concerning patient outcomes, says lead investigator Alison Trinkoff, ScD, FAAN.

For example, pneumonia deaths were significantly more likely in hospitals where nurses reported increased psychological demands and more adverse work schedules. Trinkoff says they measured high psychological demands by very fast work, lack of time to complete work, excessive required work, being slowed by delays from other workers, and frequent interruptions. Another trend was that patients were more likely to develop deep vein thrombosis after surgery in hospitals where nurses reported high psychological demands. Staffing also was controlled in the analysis, so that the effects occurred independent of staffing.

Confessions of a Psych Nurse, Nursetogether.com

A Nurse Confesses:  There is no way to work on a psych ward of a mental hospital and not learn something about life; I have met some of the strangest and most original individuals.  When people find out where I work, and have worked for almost 22 years, their mouths hangs open in awe.  Most of the time the phrase, "I don't know how you do it" is mentioned, as they shake their heads.

 

I confess there are things about working in a mental health institution that I do not like, and there are times when I have to bite my tongue and keep my lips glued together because I become so agitated.  I thought I would list for you my dislikes and explain later what I have learned.  Deep breath…here I go.

 

I dislike when someone comes into the hospital just so they can get a check (aka crazy check) when they are clearly healthy but truly too damn lazy to work.

 

I dislike when someone is purely and simply mean spirited and uses their diagnosis of being mentally ill as an excuse to cling to.

Time to Eliminate Dangerous Injection Practices by Clinicians : Health in 30

Thanks to Laura Landro for shining light on unsafe injections in her WSJ blog, “Unsafe Injection Practices Persist Despite Education Efforts.”

Landro writes:

“A new push is underway to eliminate unsafe injection practices, which remain a persistent safety problem despite years of efforts to educate clinicians about the risks of re-using needles, syringes and drug vials.

In the U.S., failure to follow safe practices in delivering intravenous medications and injections has resulted in more than 30 outbreaks of infectious disease including hepatitis C, and the notification of more than 125,000 patients about potential exposure just in the last decade, according to health-care purchasing alliance Premier Inc.”

As a registered nurse this is unthinkable.  Learning to administer injections safely is “patient care 101.”  There is no excuse for any health care professional to unsafely inject patients.

More Information on This Aging in Place Idea « Design With the Future In Mind

Over the last little while, I have been filling you in on Elsie and John, a senior couple who live in their fifty year old home but are choosing to look for a home that they can live safely and comfortably in through their mature senior years. 

Recently, while visiting with their friends Tilly and Harold, another mature senior couple, they discussed their search. Harold mentioned he has been seeing more and more information on this Aging in Place idea.

John explained how Aging in Place is making the news lately because so many homeowners are in the same boat as they are: their home is nice to look at but it just isn’t functional for them as they are becoming more mature seniors. Remodelers, builders and homeowners are beginning to take notice of the need to prepare the homes for ease of living when seniors reach their later years. He further stressed that they were not interested in a home that felt institutional but it did need to be safe, comfortable and low maintenance.
