Americans’ dislike for health insurers is a trending topic.
Seventy percent of social media comments about major health insurers were negative during the past year, according to a new report released by Amplicate, a social media analytics service. The company analyzed public tweets, Facebook posts and comments on its own website between September 2010 and August 2011. Social media users made 2,311 comments total about 18 insurers, and those opinions were shared more than 632,000 times. Insurers aren’t alone, and they aren’t the most talked about: In the past year, U.S airlines generated over 34,000 comments and U.S. banks had nearly 22,000 comments.

Rising health care costs put focus on illegal immigrants - USATODAY.com
Juan Perez had stomach pains for a month before deciding to visit a health clinic here that is open Thursday nights so migrant farmworkers don't miss a day working in the fields.As an illegal, uninsured immigrant, Perez has had problems in the past — not only with his health, but with navigating the U.S. health care maze. In Michigan, there was no interpreter at his local health clinic, the bills had to be paid in installments, and co-workers warned that a visit to a doctor could lead to deportation. In North Carolina, he's found a health care home at Tiffany Revels' weekly clinic — providing he can hitch a ride there.
"The biggest concern is getting sick, because you don't have anyone here," Perez says after Revels, a family nurse practitioner at the federally funded clinic, prescribes two antibiotics and Pepto-Bismol tablets for his bacterial gastritis. "You are here by yourself."

More working-age Americans are going without health insurance and not seeking physician care for injuries or illness because they can't afford it, according to two new studies released in March.
A report by the New York-based Commonwealth Fund found that the portion of patients delaying medical treatment in the last year is trending upward. Findings from the group's biennial health insurance survey in 2010 show that an increasing percentage of working-age adults skipped office visits, medical tests and prescriptions because of costs.
Many survey respondents are going without health coverage after losing a job during the economic recession, said Sara Collins, an author of the Commonwealth Fund study.

Governors: Medicaid More A Budget Buster Than Ever : NPR
The federal government and the states have shared the cost of Medicaid, the health insurance program for some 60 million low-income Americans, since it was created in 1965.
They've shared something else almost that long — arguments about who should foot how much of the ever-escalating bill.
"Medicaid cost growth has been a problem for time immemorial," says Alan Weil, executive director of the National Academy for State Health Policy.
But this time, he says, things are different.
For one thing, "the program is bigger, so growth on a larger base is more real dollars that's harder to find."

Released hospital patients' many unhappy returns
Patients who are released from the hospital too early or without proper planning and instructions often wind up back in the hospital after a few days, a problem that's costly to taxpayers and distressing to patients.
A study released today calculated that reducing hospital stays by a single day for Medicare and Medi-Cal patients in California adds up to $227 million a year.
An estimated 81,000 Medicare patients in California - or 20 percent - end up back in the hospital within 30 days of being discharged for some reason related to the same condition, the study found.
"Right now, when you go to the hospital, it's the do-it-yourself model. It's up to you to figure out what to do," said David Grant, author of the study for the California Discharge Planning Collaborative, a group of labor, senior and other advocacy organizations.
Patients, especially those who are elderly and lack social support, are often readmitted because they don't understand their discharge instructions, fail to take their medications or have complications that they can't handle.

Hospital care easier, faster with standing orders - CNN.com
My patient one day, a spry 80-year-old, started to cough and feel short of breath during a blood transfusion: classic signs of a transfusion reaction. I stopped her IV, but she needed a steroid to bring her breathing back to normal.
Unable to reach her primary physician, we called in a rapid-response team. An ICU doctor, respiratory therapist, two ICU nurses, a nurse anesthetist, and MDs and RNs from the floor all rushed into the room . . . . to authorize giving my patient this one needed drug.
The patient did not need rescuing, just a dose of solumedrol, and I could have given her that dose, without wasting the time and energy of multiple nurses and doctors, if we had a protocol, or "standing order," in place in my hospital for treating transfusion reactions.
A standing order is a kind of treatment algorithm used in hospitals to expedite care. Protocols are designed by doctors and nurses, implemented by nurses, and are typically used either in specific emergencies or to deliver routine care. A protocol for treating low blood sugar is an example of treating an emergency; putting silver nitrate in a newborn's eyes counts as routine.
Protocols make a lot of sense, according to Nancy Foster, vice president for Quality and Safety Policy for the American Hospital Association. The AHA supports the use of standing orders because, Foster says, "Standardization is an effective way to make sure we do the right thing for the right patient at the right time."

Opposition to health-care rationing is a little like opposition to growing up. It sounds great. It’s just not very practical.
A society’s resources are always limited. So we have to make choices about what we can afford and what we can’t. Not everyone can afford to own a vacation home — which means vacation homes are rationed. Not everyone can afford to live in towns with excellent public schools — which means that good public education is rationed.
Similarly, we can’t afford to try every feasible medical treatment on every patient. Instead, we make choices. The most obvious form of rationing is the millions of Americans who lack health insurance today. Most of them get less medical care than they need and, in the process, keep down the nation’s total medical bill.
But even those with health insurance experience rationing. How? In many ways.
This country has not spent the money to install computerized medical records, and we suffer more medical errors than many other countries. We underpay primary care doctors, relative to specialists, and we’re left stewing in waiting rooms while our primary-care doctors try to see as many patients as possible. Specialists are usually not paid for time they spend collaborating with doctors in other specialties, and many hard-to-diagnose conditions go untreated. Nurses are usually not paid to counsel people on how to improve their diets or remember to take their pills, and manageable cases of diabetes and heart disease become fatal.
At some point we'll need to accept the fact that we cannot all have every test, every operation, every treatment, every new medicine or see every specialist.
