Health Experts & Patient Advocates Beware: 10 Reasons Why you Shouldn’t be a Curator at Organized Wisdom!! #OrganizedWisdom « Laika's MedLibLog

Organized Wisdom shares health links of health experts or advocates, who (according to OW’s FAQ), either requested a profile or were recommended by OW’s Medical Review Board. I was one of those so called Expert Curators. However, I had never requested a profile and I seriously doubt whether someone from the a medical board had actually read any of my tweets or my blog posts.

This was one of the many issues with Organized Wisdom. But the main issue was its lack of credibility and transparency. I vented my complaints, I removed my profile from OW, stopped following updates at Twitter and informed some fellow curators.

I almost forgot about it, till Simon Sikorski, MD, commented at my blog, informing me that my complaints hadn’t been fully addressed and convincing me things were even worse than I thought.

Research and regulatory bodies have long confirmed that poor communication in healthcare is harmful at best and deadly at worst.

In the 2005 study, Silence Kills, VitalSmarts and the AACN found that 84 percent of healthcare professionals observe colleagues take dangerous shortcuts when working with patients and yet less than 10 percent speak up about their concerns.

Since that time, the healthcare community has turned to safety tools and checklists to reduce unintentional slips and errors. And yet, a new study called The Silent Treatment has found that the effectiveness of safety tools is undercut by undiscussables. Every day, healthcare professionals are making calculated decisions to not speak up—even when safety tools alert them to potential harm.

The Silent Treatment reveals that despite the safety interventions taken in the last decade, silence still kills. Safety tools do not compensate for crucial conversations failures in the hospital.

When a person is told that they have a serious illness, they are similar to Alice falling down the rabbit hole.

They enter a bewildering new world of discussions, tests and treatment programs that must be navigated while maintaining a job, life obligations, and relationships with friends and family. Just when a person needs more resilience to stress, anxiety can occur. The ability to function is often diminished in such a state, and, like Alice, many begin to grope for solutions. Patients, family members and friends begin to rely on anecdotes. Others find hope-based messages more acceptable than evidence-based ones. Some patients are willing to do anything that appears constructive even if there is no evidence of success.

Some patients seek to become experts in their field of illness and venture on-line to do so, but entering the world of medicine on-line often creates misguided but nonetheless strongly held beliefs. Much of the accessible material is jargon and advertising rather than unbiased information. Web sites and brochures are rife with marketing language to promote a hospital, company or pharmaceutical product, leaving many who seek information as wary consumers instead of informed patients.

No matter what health-care reform winds up looking like, one thing’s certain: Nurses will play a bigger role than ever, taking over duties from doctors, moving out of hospitals and into the community, looking after frail aging boomers, leading the push to keep costs down and improve outcomes. So this seemed the perfect time for a consult with the people who really run health care in Philadelphia. As one nurse told us, “Doctors only think they’re the quarterbacks.” Here’s what nurses had to say about their work, their patients, life and death, and those little white caps they used to wear.

1 // It’s okay to buzz // Really. Go ahead. Room too cold? Need a pillow? Got a question about your meds? Use the buzzer; that’s why they put it there. “My 82-year-old mother was just in the hospital for a stroke,” says Presbyterian Hospital’s Michael Becker, “and she said, ‘I can only hit the buzzer two times a day.’ I asked, ‘Why is that?’ and she said, ‘Because they’re going to get tired of seeing me.’ I said, “No, no, Mom, it doesn’t work that way.’” If somebody’s pushing the buzzer all the time, nurses know there’s a deeper issue and work to address it.

2 // You don’t have to be afraid // Nurses understand that patients often don’t speak up because they feel vulnerable. But when we don’t voice our thoughts, they have no way of knowing what needs improvement—until the patient-satisfaction survey arrives after the fact. And they hate finding out there was something they could have fixed if they’d known about it. “It’s natural to be fearful,” says Pennsylvania Hospital’s Mary Del Guidice, “because you’re lying in bed with all these faces looking down at you. But don’t be afraid of us.”

Impetus for the innovation came after the well-publicized medical error that ended in the death of 18-month-old Josie King at Johns Hopkins in Baltimore, Md., and the resulting focus on patient and family involvement in acute care.

Although patients and families can use the system to alert staff about potential emergency situations, such as chest pain, it also offers the reassurance that help is nearby. This is in case patients experience delays in bedside care, pain medications, and more, according to Ginni Norton, RN, MS, Nursing Performance Improvement, Nyack Hospital, a member of the NewYork-Presbyterian Healthcare System.

A team approach

Nyack Hospital patients and their visitors can call 3-1-1-1, which goes directly to hospital operators. The operators have been trained to ask callers questions according to an algorithm. Callers who report something physiological, such as bleeding or chest pain, are be routed immediately to rapid response. The call can trigger a team, including the nurse manager from the patient's unit, house physician, respiratory therapist, critical care nurse, and pharmacist.

The Need for Spinal Injury Recovery Foundation

A family never plans on having a member severely injure their spine. While in intensive care, families face the extreme shock of suddenly having to arrange for long-term rehabilitation and around-the-clock care for their loved one. In Las Vegas, these families currently have little or no resources available to guide them through a myriad of crucial decisions that will impact the outcome of their loved one's long-term recovery.

Founder's Story

No one knows this better than Scott Frost, founder of SIRF, father, fiancé, loving son and entrepreneur. On May 10, 2009, Scott was injured in an off-road dirt bike accident that damaged his spinal cord and left him paralyzed from the neck down. What started as a day of fun in the hills south of Henderson Executive Airport turned into a near-fatal chain of events. [Read Scott's complete story here.]

SIRF's Origin

During the 70 days Scott spent in Las Vegas' Desert Canyon Rehabilitation Hospital, he watched other patients who didn't have the help of friends and family like he had. Many had given up hope for their futures. "It made me furious that this could happen in this day and age." I don't want anyone to have to go through the hopelessness of being so alone and distraught. I was surrounded by loved ones. I can't imagine if I hadn't had that.
