Time to Eliminate Dangerous Injection Practices by Clinicians : Health in 30

Thanks to Laura Landro for shining light on unsafe injections in her WSJ blog, “Unsafe Injection Practices Persist Despite Education Efforts.”

Landro writes:

“A new push is underway to eliminate unsafe injection practices, which remain a persistent safety problem despite years of efforts to educate clinicians about the risks of re-using needles, syringes and drug vials.

In the U.S., failure to follow safe practices in delivering intravenous medications and injections has resulted in more than 30 outbreaks of infectious disease including hepatitis C, and the notification of more than 125,000 patients about potential exposure just in the last decade, according to health-care purchasing alliance Premier Inc.”

As a registered nurse this is unthinkable.  Learning to administer injections safely is “patient care 101.”  There is no excuse for any health care professional to unsafely inject patients.

    The Phlebitis Scale does mean something.. | Infusion Nurse Blog

    Nurses know that phlebitis is the inflammation of the vein and is considered an adverse patient outcome. Vascular access sites should be routinely assessed for signs and symptoms of phlebitis , as well as the severity of phlebitis. According to the Infusion Nurses Society(INS) Standards of Practice, a standardized scale that is valid, reliable, and clinically feasible should be used.  For the adult population, two phlebitis scale have demonstrated validity and reliability.

    One of the two scales is the phlebitis scale from the Infusion Nurses Society that many infusion nurses have used for years.

    Phlebitis Scale (from the Infusion Nursing Standards of Practice 2011 S47)
    Grade 0 – No symptoms
    Grade 1 – Erythema at access site with or without pain
    Grade 2 – Pain at access site with erythema and/or edema
    Grade 3 – Pain at access site with erythema and/or edema, streak formation, palpable venous cord.
    Grade 4 – Pain at access site with erythema and/or edema, streak formation, palpable venous cord greater than 1 in in length; purulent drainage.

    China Too Connected to IV Meds - Nursing Link

    Walk into any hospital or clinic in China, especially in winter, and the crowds connected to IV drips swiftly confirm the worries of experts here that China is overdosing on IV meds, often for minor ailments.

    Per capita consumption of antibiotics, commonly given by IV rather than the cheaper method of pills or injections, is 10 times the world average, said Wu Zhen, vice director of the State Food and Drug Administration, in December, the state-run Xinhua News Agency reported.

    Watts, H. David : Starting the I.V.

    This poem explores the act of inserting an intravenous line (I.V.) into a patient just prior to induction of anesthesia or sedation. The physician-narrator is initially full of bravado, stating "I am good at this" and "I'm the best". The physicality of the act is detailed: the vein "lies stretched and succulent" and the needle "waits / like a mosquito attached / by its sucker." By the end of the second stanza, however, when the I.V. has been successfully inserted, the significance of this seemingly simple medical intervention is stated: "I am suddenly aware / I am connected to his brain.
