Googling Symptoms: How It Can Help Patients and Doctors - TIME

But to debate whether patients should or should not Google their symptoms (which a surprising number of doctors seem to enjoy engaging in) is an absurd exercise. Patients already are doing it, it is now a fact of normal patient behavior, and it will only increase as Internet technology becomes ever more ubiquitous. The average Joe has more health information at his fingertips — both credible and charlatan — than all the medical libraries ever built put together. So the real question is, What can professionals do to translate this phenomenon into better health for their patients and the public?

First, they can adopt strategies in health-care delivery and education that endorse the process of patient self-education. Shared clinical decisionmaking is an influential model for patient care now being promoted by researchers, educators and the federal government as a way to get patients to partner with their doctors to take an active role in making decisions about health care. The ideas behind this model were outlined in the late 1990s by researchers at McMaster University in Ontario, Canada, and were important enough to be included in the language of both the federal stimulus and health-reform laws. This model recognizes that many choices in medical care often involve complicated trade-offs. In this process, patients are encouraged to become informed of the nuances related to a health-care decision in advance of the doctor-patient encounter; and the Internet has naturally become the place where these tools can be found in the form of worksheets, videos and decision aids. Social Media - How to Use Social Media to Choose a Doctor

We patients have only a few tools at our disposal to choose the right doctor for us. Most of those tools provide very cut and dried basics like licensing, board certification, even malpractice.

It has always been difficult to gain a glimpse into a doctor's attitudes or personality without meeting that doctor and getting to know him or her. Friends might tell us a doctor is "nice" - but smart patients know that doesn't mean a doctor is competent.

Along comes social media - Facebook, Twitter, YouTube, LinkedIn and other programs online that allow us to connect with other people - including doctors. Social media allows us to learn more about a doctor's personality and attitudes, his or her approach to their work and more.

With a little detective work, picking up clues here and there, smart patients use social medial to help them research and choose their doctors.

Some might tell you that you can get this kind of information from doctors' ratings sites. But I'm not a fan. They are too restricted, and are too often populated only by disgruntled patients. So give social media a try, hopefully to find more balance.

Patients find plenty of health information on line, but not all of it is reliable

The Internet has no equal as an information storehouse. The trick is to know how to get right to a source of useful information and not waste time on Web sites that are biased, trying to sell you something or just plain wrong.

Marvin M. Lipman, Consumers Union's chief medical adviser, recalls having a patient who made a Google search and somehow settled on an abdominal aortic aneurysm (a worrisome bulge in the body's main blood vessel) as the logical explanation for his midback pain. No reassuring on Lipman's part eased the patient's apprehension. It took a sonogram to convince him he wasn't at death's door.

Lipman had another patient who was referred to him after her primary-care physician told her she had Graves' disease (an overactive thyroid). She arrived for her appointment armed with computer printouts of useful, accurate information and fully prepared to discuss the pros and cons of treatment options for her problem.

Teenage patients are answering medical questions on Yahoo! Answers

Doctors debate whether patients Googling symptoms is a good thing or a bad thing. I bet you haven’t thought about something that might be worse:  your teenage patients are answering one another’s medical questions on Yahoo! Answers.

They’re not always clear on anatomy and terminology:  “How can I tell if I’m circumcised?” and “What’s the best way to shave my vagina?” and “Where is the female prostate located?”

They are sexually active and not well informed: “If my boyfriend and I put the condom on after we start, can I still get an STD?” and “I had the HPV shot, am I protected from other STDs and AIDS?” and “I’m 12, should I take Viagra for my penis?”

They are worried that they are too tall, too short, too heavy, too thin, that their parts are the wrong shape or size. They believe every fad diet in the world. “Is anorexia a good diet?” and “Is it healthy to drink 16.9 fluid ounces of water per day?”

Hello Doctor! (Can I Call You M.D.?) - Health Blog - WSJ

The medical profession feels like it is under siege. The traditional MD trained, post residency doctor was trusted and looked up to as a pillar of knowledge and achievement.
In a vast array of ways there have been efforts to dislodge these professionals from their perch. Obfuscation of roles, privilege creep, payer acceptance, degree inflation, DTC advertising along with mid-level focused detailing(generally more corruptible), push of “universal health care”, push of generic terminology such as “providers” used in ways to commoditize then devalue the services. The push of EHR and protocol driven care, etc are all parts of the efforts to this end.
The problem is that this is leading to fall in standards, unfortunately not just in affiliated providers but even amongst MDs in my observation over the years. The approach of younger doctors appears to be that “if I am merely a provider I will just show that level of commitment” . Many no longer take out of hours call. They choose lifestyle specialties, prefer shift work, generally refuse anything more than minimal responsibility etc.
The old guard is aging and as they are phased out, the true unfortunate ramifications of this will become clear. We are being very short sighted undercutting such a venerable profession in such ways.

First aid rules that everyone needs to know, Alax Lickerman, MD, KevinMD.com

First aid is defined as the immediate care given to an acutely injured or ill person.  It can literally be life-saving so it behooves all of us to know some basic principles.

What follows are some rules that cover common conditions and general practices:

  1. Don’t panic.  Panic clouds thinking and causes mistakes.  When I was an intern and learning what to do when confronted with an unresponsive patient, a wise resident advised me when entering a “code blue” situation to always “take my own pulse first.”  In other words, I needed to calm myself before attempting to intervene.  It’s far easier to do this when you know what you’re doing, but even if you encounter a situation for which you’re unprepared, there’s usually some good you can do.  Focus on that rather than on allowing yourself an unhelpful emotional response.  You can let yourself feel whatever you need to feel later when you’re no longer needed.
  2. First, do no harm.  This doesn’t mean do nothing.  It means make sure that if you’re going to do something you’re confident it won’t make matters worse.  If you’re not sure about the risk of harm of a particular intervention, don’t do it.  So don’t move a trauma victim, especially an unconscious one, unless not moving them puts them at great risk (and by the way, cars rarely explode).  Don’t remove an embedded object (like a knife or nail) as you may precipitate more harm (e.g., increased bleeding).  And if there’s nothing you can think to do yourself, you can always call for help.  In fact, if you’re alone and your only means to do that is to leave the victim, then leave the victim.
  3. CPR can be life-sustaining.  But most people do it wrong.  First, studies suggest no survival advantage when bystanders deliver breaths to victims compared to when they only do chest compressions.  Second, most people don’t compress deeply enough or perform compressions quickly enough.  You really need to indent the chest and should aim for 100 compressions per minute.  That’s more than 1 compression per second.  If you’re doing it right, CPR should wear you out.  Also, know that CPR doesn’t reverse ventricular fibrillation, the most common cause of unconsciousness in a patient suffering from a heart attack.  Either electricity (meaning defibrillation) or medication is required for that.  But CPR is a bridge that keeps vital organs oxygenated until paramedics arrive.  Which is why…
  4. Time counts.  The technology we now have to treat two of the most common and devastating medical problems in America, heart attacks and strokes, has evolved to an amazing degree, but patients often do poorly because they don’t gain access to that technology in time.  The risk of dying from a heart attack, for example, is greatest in the first 30 minutes after symptoms begin.  By the time most people even admit to themselves the chest pain they’re feeling could be related to their heart, they’ve usually passed that critical juncture.  If you or someone you know has risk factors for heart disease and starts experiencing chest pain, resist the urge to write it off.  Get to the nearest emergency room as quickly as you can.  If someone develops focal weakness of their face, legs, or arms, or difficulty with speech or smiling, they may be having a stroke, which represents a true emergency.  Current protocols for treatment depend on the length of time symptoms have been present.  The shorter that time, the more likely the best therapies can be applied.
  5. Don’t use hydrogen peroxide on cuts or open wounds.  It’s more irritating to tissue than it is helpful.  Soap and water and some kind of bandage are best.
  6. When someone passes out but continues breathing and has a good pulse, the two most useful pieces of information to help doctors figure out what happened are:  1) the pulse rate, and 2) the length of time it takes for consciousness to return.
  7. High blood pressure is rarely acutely dangerous.  First, high blood pressure is a normal and appropriate response to exercise, stress, fear, and pain.  Many patients I follow for high blood pressure begin panicking when their readings start to come in higher.  But the damage high blood pressure does to the human body takes place over years to decades.  There is such a thing as a hypertensive emergency, when the blood pressure is higher than around 200/120, but it’s quite rare to see readings that high, and even then, in the absence of symptoms (headache, visual disturbances, nausea, confusion) it’s considered a hypertensive urgency, meaning you have 24 hours to get the pressure down before you get into trouble.
  8. If a person can talk or cough, their airway is open.  Meaning they’re not choking.  Don’t Heimlich someone who says to you, “I’m choking.”
  9. Most seizures are not emergencies.  The greatest danger posed to someone having a seizure is injury from unrestrained forceful muscular contractions.  Don’t attempt to move a seizing person’s tongue.  Don’t worry—they won’t swallow it.  Move any objects on which they may hurt themselves away from the area (including glasses from their head) and time the seizure.  A true seizure is often followed by a period of confusion called “post-ictal confusion.”  Your reassurance during this period that they’re okay is the appropriate therapy.
  10. Drowning doesn’t look like what you think it does.  For one thing, drowning people are physiologically incapable of crying out for help.  In fact, someone actually drowning is usually barely moving at all (I strongly encourage everyone to click on this link to learn more about how to recognize what drowning does look like).

More Than Half of Texans Lie About Health and Fitness Habits

Survey Finds Women More Dishonest Than Men and Correlation Between Weight and Lying

DALLAS (January 31, 2011) – At a time when many people are working to lose weight and get healthier, more than half of Texans admit to lying to family or doctors about their health and fitness habits such as nutrition and amount of exercise, according to the True Results Health Honesty Survey.  Forty-six percent are not honest with family members and 32 percent admit to lying to doctors.  True Results is a team of leading weight loss experts based in Texas.

The True Results Health Honesty Survey reveals more than half of Texans lie about their health and fitness habits, women are more dishonest than men and the rate of lying is correlated to weight.Of those that lie, the majority (70 percent) do so only a few times per year and the main reasons for lying are embarrassment about one’s real habits (57 percent) and not wanting to explain the truth (33 percent).  Only 30 percent have been confronted about their dishonesty.

“Lying about your health and fitness, if even only a few times per year, can signify a fundamental issue in your ability to achieve your health goals,” said Jessica Diaz, nutritionist and exercise physiologist for True Results.  “Numerous studies have shown, the key to achieving any health or fitness goal is support from those around you and that cannot happen if you’re not honest with yourself or others.”

Gender and Weight Play a Role

True Results’ survey also revealed that slightly more women lie about their health and fitness habits to family than men (50 percent versus 43 percent) and to their doctors (34 percent versus 28 percent.)  The poll also showed that overweight people are less honest with others.  Weight categories were calculated by determining each respondent’s Body Mass Index (BMI), or measure of body fat based on height and weight.

OrganizedWisdom Finds Free Experts Make for SEO Success - NYTimes.com

With Demand Media currently planning a high-profile IPO and issues being raised about the impact of “content farms” on search results, there is a lot of attention being paid to the question of content aggregation and its effect on the web. OrganizedWisdom Health, a health-resource service, now believes it’s better — and more profitable — to organize free content rather than pay for low-wage material.

The New York-based startup has spent most of the past four years building up an index of “wisdom cards:” topical health entries that were written by contributors for a small fee. At any given moment, OrganizedWisdom contracted with 100 writers, who covered a wide array of medical topics. But exactly one year ago, the company dropped the paid-content route and embraced a free expert model. So where does the content come from? The startup keeps track of some 5,000 doctors and health experts who use social media, and collects their contributions into wisdom cards.

Internet in Medicine: 2000 vs 2010 « ScienceRoll

Whenever I talk about using social media in medicine to doctors, they seem to think there are more cons than pros regarding this issue. Well, I like reminding them about some major differences between 2000 and nowadays.

What would I do if in 2000 Now
I need clinical answer Try to find a collegue who knows it Post a question on Twitter
I want to hear patient story about a specific condition Try to find a patient in my town Read blogs, watch Youtube
I want to be up-to-date Go to the library once a week Use RSS and follow hundreds of journals
I want to work on a manuscript with my team We gather around the table Use Google Docs without geographical limits

Think you can ignore social media in your practice and workplace? Maybe. However you could embrace, utilize it and enhance your performance too.

More Patients Making Appointments Online As Doctors Embrace Web - Kaiser Health News

After relocating to Washington, D.C., Clint Morrison needed a doctor to follow up a tonsillectomy he had a few weeks earlier in California. But when he started calling specialists in his health plan’s directory, he struck out: they either weren't taking new patients or had no openings for several weeks.  

So in September Morrison went to a website called www.zocdoc.com. He could see doctors' appointment calendars and identify those that took his insurance and were located near his office. With a couple clicks, Morrison, 24, scheduled an appointment for the next day with Mark Dettelbach, an ear, nose and throat doctor. "It was painless," Morrison says of the experience.

Dettelbach's group practice is one of a growing number that allow patients to schedule appointments online — eliminating multiple calls, waiting on hold and increasing the likelihood of finding someone with an opening.

About 16 percent of family doctors used online scheduling in 2009, up from 6 percent in 2005, according to the American Academy of Family Physicians. Most are doing it on their own or through health systems where they work.

Several factors drive the trend. On the doctors' side, it's mostly the need to add patients and reduce overhead costs. They're also growing more comfortable with computers. About half of family doctors use electronic health records and 44 percent prescribe electronically, according to the academy.
