Why the day after Christmas is hazardous to your heart - CNN.com

December 26 is historically one of the most dangerous days of the year for people vulnerable to cardiac problems, including heart attacks, arrhythmias, and heart failure.

And many of these so-called Merry Christmas coronaries will hit people who didn't even realize they were at risk when they unwrapped their gifts the day before.

But the holiday season isn't good for heart health to begin with. A 2004 study by researchers at the University of California, San Diego and Tufts University found that heart-related deaths increase by nearly 5% during the holidays, perhaps because patients delay seeking treatment for heart problems or because hospital staffing patterns change.

But anecdotally, doctors say that their ERs stay quiet on Christmas Day itself. Then, come December 26, they see a surge of cardiac traffic. A 2008 study found that daily visits to hospitals for heart failure increased by 33% during the four days after Christmas.

Many local nursing grads find local jobs, Utah

A couple of years ago, finding a job as a nurse was a snap.

It's a bit harder now, but thanks to a healthy relationship between the hospitals in Utah County and the local nursing schools, local grads are still doing OK.

Utah had the lowest registered nurse vacancy rate in the country as of May, said David Clark, Intermountain's CEO for the urban south region. Much of that he attributed to the natural feeding system set up because of BYU, UVU, Provo College, Eagle Gate College, Ameritech and other programs that train qualified nurses.

Intermountain Healthcare is the largest health care provider in the state and thus the largest employer of nurses as well.

And he has no problem admitting that Intermountain is a little partial to the nurses who trained "right there in our backyard."

It's not that the process becomes easy, he said.

"They have to earn it based on their own merits," Clark said.

6 Tips to Survive Your First Year as a Hospital RN - Nursing Link

The first year on the job is often the toughest for new nursing graduates, especially those who work in hospitals. In fact, new nurse graduates account for more than half of the turnover rate in some hospitals, according to a study published in 2007 by Johns Hopkins University School of Nursing researchers.

“There really are multiple reasons for [the first-year exodus],” says Patricia Benner, RN, PhD, professor at the University of California, San Francisco and a senior scholar at the Carnegie Foundation for the Advancement of Teaching. “One is that nursing practice is incredibly complex. Over the past 60 years, the transfer of responsibility to nursing from medicine has been incredible. I think society doesn’t typically recognize that.”

Because the sickest patients are in the hospital, hospital RNs need good clinical judgment and the ability to recognize when a patient needs immediate intervention — challenges that are especially pronounced in a nurse’s first year of employment.

Despite the Odds, I Became a Nurse, Nursetogether.com

Teen mothers are more likely to drop out of high school than girls who wait to have children.

Statistics will tell you that only 40 percent of teenagers who have children before age 18 move forward and graduate from high school.  Statistics also say that only 1.5% will have a college degree by age 30.*  Teen mothers are more likely to end up on welfare (nearly 80 percent of unmarried teen mothers end up on welfare according to the March of Dimes) and will wind up living in poverty with their children.**

Glad I didn’t pay much attention to statistics.

Despite becoming pregnant as a teenager, I didn't drop out of high school; I graduated early, enrolled in college and became a nurse.

Business Basics, Starting A Business, Initial Steps, NursingEntrepreneurs.com

Steps To Start Any Business, in Any State (This example is for a nursing agency):

  • Develop a Business Plan.

    Small Business Administration:"What goes in a business plan? This is an excellent question. And, it is one that many new and potential small business owners should ask, but oftentimes don't ask. The body of the business plan can be divided into four distinct sections: 1) the description of the business, 2) the marketing plan, 3) the financial management plan and 4) the management plan. Addenda to the business plan should include the executive summary, supporting documents and financial projections."

    • What is your product or Service? (Certified Nursing Assistants (CNAs), Licensed Practical/Vocational Nurses (LPN/LVNs), Registered Nurses (RNs)) providing nursing care.

    • Who will you provide it to? (Homecare patients, Hospitals, Nursing Homes, Small Businesses, Large Corporations).

      • When deciding whether to service Hospitals or Nursing Homes, remember that:
        • Hospitals will use more Registered Nurses (more highly skilled and can take on more responsibilities).
        • Nursing Homes will used more Licensed Practical/Vocational Nurses (it's much cheaper to use them for passing medications and doing treatments on a large scale-most RNs in Nursing Homes function as administrative or charge nurses).

    • What will you Charge?

    • Where will you operate from?

    • How will your fund your enterprise, cover Start-up Costs (Borrow Money, Use Existing Funds).

  • Naming Your Business-Choose a name that will represent you and the services you intend to provide.

  • Optional: Choosing or Designing a logo: My personal feelings on this are, its appropriate for Nike or Visa, but for a small business just starting out it is expensive (a good logo can run you a few hundred to a few thousand dollars) and is not absolutely necessary. You can always create one later as you grow larger. Nursefriendly, Inc. does not have a logo, and doesn't foresee a need for one in the near future.

  • Decide What type of legal business organization you'd like. Costs of registering a sole proprietorship, partnership or incorporation ($50-$1,000). Typically you don't need an Attorney to Set up your business, though it will streamline the process and increase the cost.

    I'd compare setting up your business to doing your taxes.

    Most people can do their taxes themselves, fill out the tedious forms and pull their hair out. Or you can pay an accountant to do them who will be happy to send you a large bill for his services.

    When I first talked to my Attorney about Incorporating, he quoted me $995 to have his office take care of it for me.

    When I called the Department of Taxation & Revenue in New Jersey, they told me it would cost $125 in filing fees and $25 for the name search. . . . . . . so by filling out the forms myself, I saved close to $800.

When starting a new nurse registry business, you are entering into a legal business environment that requires you to take steps to protect your personal assets, such as your home, your savings, and your retirement. With this in mind, you need to determine which legal form you are going to take for your new registry. Some of the common choices are a sole proprietorship, a partnership, a limited liability partnership (LLP), a limited liability company (LLC), and a corporation."

    It's mostly a matter of convenience, and whether you've got more time or money at this point.

    The basic types of business organizations you can consider are:

    • Sole Proprietorship (all income is reported under your social security number, and taxable as personable income). All liability in this case falls on your shoulders and creditors can go after your personal assets. A single award in a medical malpractice case (not to mention the legal bills) could devastate your business and financial assets. For a nursing agency, this is NOT recommended.

    • Partnership (same as Sole Proprietor, except there are two principal owners). Liability falls on both partners. Decision-making, Profits and Risk are shared between all partners.

    • Limited Liability Corporation. Allows for some protection of personal assets. Avoids Double Taxation inherent in a Corporation. Please note, with most small corporations, appropriate deductions can minimize your taxes and eliminate the Double Taxation Issue. For this reason, and also be cause Corporations are well defined by the IRS in terms of what deductions, benefits you can offer on a tax deductible basis, my recommendation is that you form a Full Corporation (C or S election is another issue).

  • For Profit Corporation (C or S election). Best option for tax and liability purposes. A corporation is a separate entity with it's own Tax ID (Employer Identification #). In a malpractice award, only the assets of the corporation can be touched. This option is documentation intensive.

  • Decide on Office Space, if you do a home office, costs are minimal you just need a set space and two available phone lines for the agency phone/fax. The best option here, is to form a corporation, and have the corporation lease space in your home or apartment, we can share the lease agreement (rental contract) we use with you. As part of our lease agreement, we've made Nursefriendly, Inc. responsible for bills we used to pay out of pocket.

      One point I'd like to make here is that there are a several household monthly bills your business could potentially pay and write off as tax deductible. To carry our house we pay $2000-4000/mo in basic bills. It is preferable to have the business pay bills in pretax dollars.

    • When I make a thousand dollars, I have about $700 to spend after taxes.
    • When Nursefriendly, Inc. makes a thousand dollars, no taxes are taken out initially, it can spend a $1000.
    • Nursefriendly Inc. only pays taxes on net profit.
    • If it has a large number of deductible expenses, which it pays in "pretax" dollars, it reduces the amount of taxes it will have to pay to the IRS.

        For Example:

    • If Nursefriendly Inc. makes $100,000 in a year (reported to it's employer identification#), and has $50,000 in deductible expenses, it only pays taxes on $50,000 at the end of the fiscal year.

    • If Andrew Lopez, RN makes $100,000 in a year (reported to his social security #), he pays taxes on the full $100,000 as checks are cut (meaning he actually has $70,000 to spend) and then hopes for a large refund at the end of the year.

  • See An Accountant: What you'll want to do is make up a list of the bills you pay monthly and sit down with an accountant. Tell the accountant that you'd like to start a business and would like to know what bills the business might be able to pay and take a tax deduction for.

    This becomes important when deciding to run a business as a Sole Proprietorship, LLC or Corporation. There are "deductions" that the IRS will question/scrutinize more heavily in a "home office" than it will in a "corporate lease."

      Some that can be paid for and deducted by your business include:

    • Association Fees (We live in a townhouse)
    • Cellular Telephones (As an agency and Internet Business, we keep in constant contact).
    • Cleaning Service, we have a service come in once every two weeks to clean our bathrooms, dust, mop, vacuum, etc. (all the housework that would take us hours to do).
    • Computer Leases (The nursefriendly leases two of its computers for $200/mo)
    • Electric ($100-300/mo)
    • Gas ($50-150/mo)
    • Health Insurance Premiums, Co-payments (When we have switched employers, we've paid $500 to $1,000/mo to maintain COBRA). Nursefriendly, Inc. covers specific medical costs for it's employees through a Medical Reimbursement Program which is fully tax deductible to the corporation.
    • Internet Access (We pay $88/mo for Comcast Cable Internet including our basic cable TV service)
    • Long Distance Service (if separate)
    • Telephones. It's expected a business will have both a voice and a fax line. ($50/mo basic service).
    • Water/Sewer ($150 per quarter).

      Some bills that your business probably cannot deduct are:

    • Automotive Maintenance & Repairs
    • Car Insurance
    • Gasoline
    • Groceries
    • Life Insurance
    • Mortage Payments

  • Set up a budget for Office Equipment, A Computer, Fax, Copier, Computer Software, as you find you need them.

  • Decide on How To Process Your Payroll (Nursefriendly, Inc. pays $45 per pay (for its first employee) through ADP Payroll services), plus payroll taxes. I've used ADP for several years. With them I can manage my payroll with a single phone call each payroll. I tell them how much to pay each employee, they cut the checks, handle deductions, taxes, deposits & tax payments.       Please note: If I pay a nurse $1000, their take home pay is round $666 (married), the government takes $334 out in taxes.

    The government also takes a matching $334 from Nursefriendly, Inc. as their employer.

    Total cost of paying an employee $1000 is $1334 to the business. Plus payroll costs. This is a cost you'll need to factor in when deciding how much to charge your facilities.

    You can have your nurses work as independent contractors to simplify this. That makes them responsible for paying their own taxes. Some nurses may not be comfortable with this as they can end up with a very large tax bill (as can you) at the end of the year.

    To Start A Nursing Agency:

  • Call The State Board of Nursing in your state, tell them you'd like to start a nursing agency. They'll refer you to the Regulating Body that can issue you a nursing agency license (Required).

    • If they do not cooperate (or say they don't know), then try calling as a nurse, who wants to file a complaint against a nursing agency and you want to know who to contact.
    • You can also try calling your local department of health and asking who licenses nursing agencies.

  • Get the required Bonding and Insurance. For Instance, in New Jersey, they require a $10,000 Surety Bond and Workers Compensation Insurance to start. Liberty Mutual handles the Involuntary Market in NJ.

  • Malpractice Insurance/Professional Liability Insurance is a must (we recommend that our nurses have individual policies as well). Health Professional Services Organization, http://www.hpso.com is reasonable, for one million/six million policy on our first 10 nurses, we pay $819 per year. Their # is 888-288-3534.

  • Pay An Attorney to make and review the contracts you'll be using with the facilities you send nurses and nursing assistants to. Please note, it's much cheaper to have an attorney "review" a contract, than to have one made up from scratch.

      (Please note, you can purchase a copy of our contracts for $50 (Visa or Mastercard Accepted) or we'll trade for competency exams, insurance quotes, etc).

  • Set up an office (either at home, or rent space if you've got the capital).

    • If you are set up as a sole proprietorship, be wary of claiming the "home office" deduction. It's a major red flag to the IRS and strict documentation rules apply.

    • If you are set up as a corporation, contact us if you'd like a copy of the Lease Agreement Nursefriendly, Inc. uses to rent office space. Strict documentation rules apply with this option as well.

  • Prepare Job Applications, Reference Sheets, and the other paperwork you'll need your for nurses to fill out for you to be in compliance with your state's requirements.

      Please note, you can find 1/2 the forms you need by applying to a local nursing agency and asking them to send you a packet so you can fill out the forms at home. (I usually tell them I'm a stay at home father and need to pay a babysitter when I come in). Send out "Request for Proposals" to printing/forms companies to get price quotes for making up your business forms, time sheets, stationery, etc.

  • Start advertising and wait for the calls, contracts to come in. Good sources are the Yellow Pages, Local Papers, Direct Mail. You can also simply mail/fax your contracts to the facilities after a phone call and let them review them.

    • Make up a list of the hospitals and nursing homes in your area (check in the local phone book, on the web, etc.) that you'll contact looking for business.

    • A guerilla marketing tactic, is to go into other facilities through other agencies working as an agency nurse, and network with other nurses and their administration.

    Everything about medicine is now big business, KevinMD

    Med­i­cine used to be different. Doc­tors couldn’t do too much for you. They didn’t get paid very much and they were focused more on helping than on managing a business.

    Hospitals were community-based not-for-profit or public entities. Drugs and devices were not as sophisticated or expensive, and they weren’t marketed directly to consumers. Well Toto, we’re not in Kansas any­more.

    After witnessing our “health­care reform” process you must have seen that almost every­thing about med­i­cine is now big business. If you don’t know that by now, you’re not paying attention.

    Yes there are still some “little guys” out there, but they’re playing by big business’ rules. What does that mean for you? Hang on, I’m coming to that.

    Now it’s often said ” the first rule of business is to stay in business.” It’s not wrong either.

    Death in a hospital is not always comfortable, KevinMD.com

    In America, too many people die in the hospital.

    I don’t mean that they die due to medical error or incompetence, though that’s always a hot topic of discussion amongst doctors, researchers, administrators, and regulators.

    What I mean is that if you ask most people, they say they’d rather die at home, surrounded by their loved ones, drifting off to sleep painlessly after having had last rites administered (feel free to plug in your religious/atheistic ritual of choice here).

    Why, then, do so many who want this type of death instead die medically, here in the hospital, undergoing painful treatment and the deprivations and degradations of medical care?

    The answer has both simple and complex aspects. But I’ll start by sharing something that most medical professionals believe:

    When my time comes, the last place I want to be is in the hospital. Don’t get me wrong–I like GlassHospital–it’s a good place to work, teach, and learn. But when the grim reaper is calling my name, I want to be as far away from here as I can.

    No IVs. No needle sticks to test my blood. No waking me up to check vital signs every shift. No hospital food. No fluorescent lights.

    No feeding tubes; no bladder catheters (ouch!); for Heaven’s sake, no breathing machines (‘mechanical ventilators’).

    Last year, Medicare paid $55 billion just for doctor and hospital bills during the last two months of patients’ lives.

    And it has been estimated that 20 to 30 percent of these medical expenses may have had no meaningful impact. Most of the bills are paid for by the federal government with few or no questions asked. This statistic is from a 60 Minutes story on “The Cost of Dying” and is one reason our healthcare system is in trouble.

    Modern medicine has become so good at keeping the terminally ill alive by treating the complications of underlying disease that the inevitable process of dying has become much harder and is often prolonged unnecessarily.  The way we set up the system right now, primary care physicians don’t have time to spend an hour with you, see how you respond, if they wanted to adjust your medication. So, the easiest thing for everybody up the stream is to admit you to the hospital. And once someone is admitted to the hospital they’re likely to be seen by a dozen or more specialists who will conduct all kinds of tests, whether they’re absolutely essential or not
