Nurses Notes: DAR and SOAP(ER) Formats - Associated Content from Yahoo! - associatedcontent.com

When it comes to nurse's notes most student nurses are up in arms on exactly what they should be writing. A focal note should be one of the easiest tasks to chart, but many students still find themselves struggling. There are different styles when it comes to writing nurses notes and you will eventually come to find a method that better suits your needs or department.

SOAP (ER) format is a common used style for nurse's notes because they are easy to understand by all medical staff. The (ER) is also included but not always applied.

Subjective Data: This section of the nurse's note should include a description of your patient based on what your patient has told you.

Example: Pain Assessment, Data that pertains to verbal statements that cannot be collected during a physical finding.

Pressure to Look Young May Be Sending More Men to Plastic Surgeons - US News and World Report

More American men seem to be lining up for facelifts, Botox injections, nose jobs and liposuction than ever before.

A new report by the American Society of Plastic Surgeons shows that the number of cosmetic procedures among men in the United States increased by 2 percent last year, compared with 2009. Men underwent more than 1.1 million cosmetic procedures, including both minimally invasive and surgical procedures, according to the report.

"Men are paying more attention to their appearance," said Dr. Phillip Haeck, the society's president.

However, the reasons that more men are having cosmetic procedures vary. "It all depends on the category," he said.

Theater owners to the FDA: Don't mess with our popcorn.

The nation's cinema operators are fuming about proposed federal rules that could require them to disclose the calories in their concession food — including popcorn, a highly profitable item for theater chains.

Regulators could issue final rules as early as Wednesday that would force movie theaters to post calorie counts for popcorn, pretzels, hot dogs and other prepared foods sold in their cinemas. Packaged foods such as candy already carry nutritional information.

A provision in the federal healthcare law stipulates that restaurant chains with at least 20 U.S. locations must provide the calorie content of menu items. Last summer, the Food and Drug Administration indicated that it intended to apply the provision not only to restaurants but also to grocery stores and concession stands, including those at theaters.

Alarmed at the prospect, representatives of the National Assn. of Theatre Owners have been lobbying the FDA and congressional staff members in recent weeks to exempt theaters from the nutritional labeling requirement.

When you can't sleep, you know it. But what about when you can, yet you wake up feeling tired and achy or you're groggy again a few hours later? What's that about? All too often, it turns out, the problem is one that doesn't keep you awake but does sabotage your sleep in more subtle ways, so the hours you spend in bed don't refresh and revitalize you the way they should. Here are seven signs that you have a sleep problem that's secretly stealing your rest.

1. You sleep poorly and wake with a bad taste in your mouth.

What it's a symptom of: "Morning mouth" can be a signal of gastroesophageal reflux disease (GERD) or asymptomatic heartburn. Recent sleep studies have shown that up to 25 percent of people who report sleeping poorly without a diagnosed cause have sleep-related acid reflux. But because they don't have obvious heartburn symptoms, they're unaware of the condition.

How it interrupts sleep: Acid reflux causes the body to partially awaken from sleep, even when there are no symptoms of heartburn. The result of this "silent reflux" is fitful, uneven sleep, but when you wake up digestion is complete and you can't tell why you slept poorly.

    Low levels of psychological distress are linked to long-term disability, study finds - latimes.com

    Even a little psychological distress adds up over time, according to a new study published online Wednesday in the Journal of Epidemiology and Community Health.

    British and Swedish researchers looked at a 2002 survey of more than 17,000 Swedes in Stockholm County, and asked them a series of questions to rate their levels of psychological distress on a scale of zero to 12. Zero means no distress, 1-2 is mild, 3-7 is moderate and 8-12 is severe.

    They then checked to see how many of these people had enrolled for disability pension awards in 2007. They found that the higher the level of psychological stress, the higher the likelihood that people would need a disability pension award five years down the line.

    Report: Communication Breakdown Leads to Hospital Errors - Health Blog - WSJ

    We’ve written about how a small but significant number of health-care workers show disrespect for colleagues, dole out verbal abuse and engage in other unacceptable behavior. Now a new report suggests this kind of poor workplace communication can also contribute to medical errors, even if other preventive steps are being taken.

    According to a two-pronged survey of operating-room and critical-care nurses conducted by their professional associations and VitalSmarts, a global training and consulting firm, 85% of 2,383 nurses surveyed said they’d been in a situation where measures put in place to reduce errors –  including checklists or hand-off protocols — warned them of a problem that would have otherwise harmed a patient.

    That’s the good news. The bad is that 58% of the nurses said they’d been in situations where it was “either unsafe to speak up or they were unable to get others to listen.”
