First aid rules that everyone needs to know, Alax Lickerman, MD, KevinMD.com

First aid is defined as the immediate care given to an acutely injured or ill person. It can literally be life-saving so it behooves all of us to know some basic principles.
What follows are some rules that cover common conditions and general practices:
Don’t panic. Panic clouds thinking and causes mistakes. When I was an intern and learning what to do when confronted with an unresponsive patient, a wise resident advised me when entering a “code blue” situation to always “take my own pulse first.” In other words, I needed to calm myself before attempting to intervene. It’s far easier to do this when you know what you’re doing, but even if you encounter a situation for which you’re unprepared, there’s usually some good you can do. Focus on that rather than on allowing yourself an unhelpful emotional response. You can let yourself feel whatever you need to feel later when you’re no longer needed.
First, do no harm. This doesn’t mean do nothing. It means make sure that if you’re going to do something you’re confident it won’t make matters worse. If you’re not sure about the risk of harm of a particular intervention, don’t do it. So don’t move a trauma victim, especially an unconscious one, unless not moving them puts them at great risk (and by the way, cars rarely explode). Don’t remove an embedded object (like a knife or nail) as you may precipitate more harm (e.g., increased bleeding). And if there’s nothing you can think to do yourself, you can always call for help. In fact, if you’re alone and your only means to do that is to leave the victim, then leave the victim.
CPR can be life-sustaining. But most people do it wrong. First, studies suggest no survival advantage when bystanders deliver breaths to victims compared to when they only do chest compressions. Second, most people don’t compress deeply enough or perform compressions quickly enough. You really need to indent the chest and should aim for 100 compressions per minute. That’s more than 1 compression per second. If you’re doing it right, CPR should wear you out. Also, know that CPR doesn’t reverse ventricular fibrillation, the most common cause of unconsciousness in a patient suffering from a heart attack. Either electricity (meaning defibrillation) or medication is required for that. But CPR is a bridge that keeps vital organs oxygenated until paramedics arrive. Which is why…
Time counts. The technology we now have to treat two of the most common and devastating medical problems in America, heart attacks and strokes, has evolved to an amazing degree, but patients often do poorly because they don’t gain access to that technology in time. The risk of dying from a heart attack, for example, is greatest in the first 30 minutes after symptoms begin. By the time most people even admit to themselves the chest pain they’re feeling could be related to their heart, they’ve usually passed that critical juncture. If you or someone you know has risk factors for heart disease and starts experiencing chest pain, resist the urge to write it off. Get to the nearest emergency room as quickly as you can. If someone develops focal weakness of their face, legs, or arms, or difficulty with speech or smiling, they may be having a stroke, which represents a true emergency. Current protocols for treatment depend on the length of time symptoms have been present. The shorter that time, the more likely the best therapies can be applied.
Don’t use hydrogen peroxide on cuts or open wounds. It’s more irritating to tissue than it is helpful. Soap and water and some kind of bandage are best.
When someone passes out but continues breathing and has a good pulse, the two most useful pieces of information to help doctors figure out what happened are: 1) the pulse rate, and 2) the length of time it takes for consciousness to return.
High blood pressure is rarely acutely dangerous. First, high blood pressure is a normal and appropriate response to exercise, stress, fear, and pain. Many patients I follow for high blood pressure begin panicking when their readings start to come in higher. But the damage high blood pressure does to the human body takes place over years to decades. There is such a thing as a hypertensive emergency, when the blood pressure is higher than around 200/120, but it’s quite rare to see readings that high, and even then, in the absence of symptoms (headache, visual disturbances, nausea, confusion) it’s considered a hypertensive urgency, meaning you have 24 hours to get the pressure down before you get into trouble.
If a person can talk or cough, their airway is open. Meaning they’re not choking. Don’t Heimlich someone who says to you, “I’m choking.”
Most seizures are not emergencies. The greatest danger posed to someone having a seizure is injury from unrestrained forceful muscular contractions. Don’t attempt to move a seizing person’s tongue. Don’t worry—they won’t swallow it. Move any objects on which they may hurt themselves away from the area (including glasses from their head) and time the seizure. A true seizure is often followed by a period of confusion called “post-ictal confusion.” Your reassurance during this period that they’re okay is the appropriate therapy.
Drowning doesn’t look like what you think it does. For one thing, drowning people are physiologically incapable of crying out for help. In fact, someone actually drowning is usually barely moving at all (I strongly encourage everyone to click on this link to learn more about how to recognize what drowning does look like).

Jeffrey L. Sturchio: The Global Burden of Cancer

Most of us in developed countries have dwelled in the shadow of cancer. We've anxiously awaited a test result, become intimate with chemotherapy for ourselves or a loved one or held vigil at a bedside.

During those intense and often tragic periods, we usually have options -- education, treatment, pain relief and sometimes, blessedly, remission and recovery -- that is, if we happen to reside in a wealthy country. Not so for millions of others, adults and children alike, in poorer countries where more than 70 percent of all cancer deaths occur yet five percent or less of cancer resources are allocated to the people living there, despite the growing cancer burden.

Cancer is the leading cause of death worldwide, killing more people than AIDS, tuberculosis and malaria combined. The cancer burden in low- and middle-income countries is increasingly disproportionate. Globally in 2009, there were an estimated 12.9 million cases of cancer, a number expected to double by 2020, with 60 percent of new cases occurring in low- and middle-income countries.

Not only do these countries carry more than half the disease burden, they lack the resources for cancer awareness and prevention, early detection, treatment or palliative options to relieve the staggering pain and human suffering if the disease is untreated -- an unthinkable outcome for people who have cancer in rich nations.

Cancer also has the most devastating economic impact of any cause of death in the world, according to the recent landmark report, "The Global Economic Cost of Cancer," released by the American Cancer Society and Livestrong. Premature deaths and disability from cancer cost the global economy nearly 1 trillion dollars a year. The data from this study provides compelling evidence that balancing the world's global health agenda to address cancer more effectively will save not only millions of lives, but also billions of dollars.

Carotid Artery Stenosis, American Heart Association

What is carotid artery stenosis or carotid artery disease?

Carotid artery stenosis is the narrowing of the carotid arteries. These are the main arteries in the neck that supply blood to the brain. Carotid artery stenosis, also called carotid artery disease, is a major risk factor for ischemic stroke. (This is the most common form of stroke and is usually caused by a blood clot plugging an artery.)

The narrowing is usually caused by plaque in a blood vessel. Plaque forms when cholesterol, fat and other substances build up in the inner lining of an artery. This process is called atherosclerosis.

How is carotid artery stenosis diagnosed?

Carotid artery stenosis may or may not cause symptoms. A doctor may hear an abnormal sound called a bruit (BROO'e) when listening to the artery with a stethoscope. The stenosis can be easily detected with an ultrasound probe placed on the side of the neck near the carotid arteries. This is called carotid ultrasonography.

How is carotid artery stenosis treated?

Depending on the degree of stenosis and the patient's overall condition, carotid artery stenosis can usually be treated with surgery. The procedure is called carotid endarterectomy. It removes the plaque that caused the carotid artery to narrow. Carotid endarterectomy has proven to benefit patients with arteries stenosed (narrowed) by 70 percent or more. For people with arteries narrowed less than 50 percent, anti-clotting medicine is usually prescribed to reduce the risk of ischemic stroke. Examples of these drugs are antiplatelet agents and anticoagulants.

Carotid angioplasty may be another treatment option. It uses balloons and/or stents to open a narrowed artery.

Patients With Blocked Carotid Arteries May Not Need Stroke Screening | National Nursing News

Patients with blocked neck arteries need screening for stroke risk only if they have other risk factors, according to new guidelines from the American Heart Association/American Stroke Association, American College of Cardiology and other groups.
A committee of stroke prevention specialists issued the guidelines after agreeing there is not sufficient evidence of benefit from widespread screening. The members hope the guidelines provide new information to help clinicians determine treatment approaches for their patients.
Screening is useful “if your doctor hears abnormal blood flow when listening to your [carotid] arteries, or if you have two or more risk factors for stroke (such as high cholesterol or a family history),” said Jonathan L. Halperin, MD, co-chairman of the writing committee and professor of medicine at the Mount Sinai School of Medicine in New York, in a news release.
Risk factors for stroke include age, family history of stroke, high blood pressure, high blood cholesterol, diabetes, obesity, atrial fibrillation, physical inactivity, sickle cell disease and other heart or blood vessel diseases.
The committee also issued recommendations regarding carotid endarterectomy and carotid stenting, two procedures for treating arteries. Both procedures are reasonable and safe, according to the committee, when arteries are more than 50% blocked.
“The guidelines support carotid surgery as a tried-and-true treatment for most patients,” said Thomas G. Brott, MD, committee co-chairman and professor of neurology and director of research at the Mayo Clinic campus in Jacksonville, Fla.
“However, for patients who have a strong preference for less invasive treatments, carotid stenting offers a safe alternative. Because of the anatomy of their arteries or other individual considerations, some patients may be more appropriate for surgery and others for stenting.”

New babies and heart attacks don't stop in a blizzard - themorningsun.com

Snowstorms don’t stop babies from being born, people from being hospitalized, and they don’t stop heart attacks.
That means that the maternity staff, hospital physicians, cardiologists and other doctors and nurses at MidMichigan Medical Center-Midland needed to plow through snow drifts or even put on skis to get to work.
Dr. Otto Leiti, an internal medicine special with MidMichigan Physicians Group, needed to get to the medical center in Midland to see his patients who were hospitalized, so he strapped on his skis and made a 40-minute ski trip to get in.
“It was fun. I had to be careful with cars, but cross country skis were designed for survival,” Leiti said. “I should do this every day.”
When maternity manager and registered nurse Tonia Van Wieren learned that the storm was coming, she decided that it would be better just to sleep over in the medical center. And when some staff could not get out to come to work, starting at 3 a.m., VanWieren was there to fill in with newly scheduled staff.
“There have been four babies delivered in the snowstorm, and eight (mothers) were in labor yesterday,” she said. “The medical center’s obstetric unit is full and overflowing on to another floor. We’re making it work.”
For some, even a snow day could not postpone the inevitable. Dr. Richard Bartling, an ral and maxillofacial surgeon who is on the Medical Center’s medical staff, was performing oral surgery at his practice in Mt. Pleasant as usual. No surgeries were cancelled and Bartling extracted teeth as usual.

Asperger’s Syndrome: Struggling to Be Next to Normal « Fern Life Center

My oldest child, Cameron, who is now 21 years old, was diagnosed with Asperger’s Syndrome (AS) when he was in 5^th grade. He is graduating next semester with a four-year college degree and an intention to proceed to graduate school. Any parent with a child on the “Autistic Spectrum” will recognize the journey that we took to get to that diagnosis and to this place. I am writing this blog to help others out there who are teachers, caregivers, family members, friends, and/or healthcare providers of these very special kids. I say special not in a sense of somehow handicapped, but special as in unique and with something to offer the world that comes through a lens of rigorous authenticity. People with AS are known for their unrelenting honesty and inability to tell even the whitest of lies to please another. What a wonderful gift for society to receive; members who have integrity that is so deeply ingrained that it is actually almost impossible to lie.

The reason I had my child evaluated was because of a parent teacher conference gone bad. Cameron’s 5^th grade teacher felt I was too “enmeshed” with him and was milking the system for the individualized educational plan (IEP) he had in place. She felt that he would function just fine without my “interference”. His IEP stipulated he was to have someone sit next to him at the front of the room that didn’t fidget or interrupt in class. That particular 5^th grade class was full of boys who had been diagnosed with Attention Deficit with Hyperactivity Disorder (ADHD) and Cameron was falling through the cracks. There had never been such a distorted ratio of boys with ADHD and so few girls in this elementary school before and the teachers were at their wits’ end trying to teach them and maintain order. Kids with AS frequently sit in class and daydream of their latest passionate interest and can get lost in the river of education as a result. I had asked for one of the quiet, steady girls to sit next to him so he wouldn’t get distracted and come home not knowing what was discussed in class.

When the teacher told me I was the problem, I thought to myself, “maybe she’s right, maybe Cameron and I should both be tested and see what is going on. Let’s face it, I AM stressed that my son doesn’t pay attention in class, is distracted on the soccer field by an ant hill, hums quietly to himself at the dinner table, and doesn’t get invited to birthday parties. Maybe reading about birds of prey, building mobiles of birds of prey, going to the Museum of Natural History in Salt Lake City for the birds of prey exhibit EVERY WEEK, making masks that represent birds of prey, doing a hawk on his birthday cake, and generally immersing myself into the world of birds of prey with Cameron is a big mistake. (Birds of prey were his current passion and he could tell you EVERYTHING about the subtlest feature of each of the birds of prey in the wild you could possibly want…and much, much, more).

CMS Initiative Will Link Incentives With Reduced Infections, Readmissions - California Healthline

CMS is planning a "major multi-year financial commitment" involving Medicare, Medicaid and private insurers that aims to curb hospital-acquired infections and readmissions, according to a confidential draft of a CMS document, Inside Health Reform reports.

The so-called National Patient Safety Initiative -- which is being developed by CMS' innovation center -- would link $70 billion in Medicare funds across 10 years to hospitals' ability to achieve new standardized performance metrics. Under the plan, 6% of hospitals' Medicare payments will be contingent on reporting errors and meeting safety measures, with the proportion of payments increasing to 9% by 2015.

By hiring state contractors, CMS will develop measures and monitor progress, and then use results to determine payments.
Medicaid and private insurance plans that chose to participate in initiative also will link a larger portion of payments to patient safety goals, affordability and patient-centered care.

The innovation center also will fund studies that aim to determine how to disseminate best practices data, and support states and health systems that develop networked learning projects, Inside Health Reform reports (Inside Health Reform, 1/26).
