Men's Health: 10 Rules of Kids' Fitness - ABC News

Parenting has never been easy. Since the beginning of time, men have tried to keep their children safe and healthy. But instead of protecting kids from, say, starvation and predators, like our cavemen ancestors, these days we're up against a modern batch of challenges: obesity and sedentary behavior—two equally formidable enemies. And since these are fairly new problems, your parents and grandparents might not have all the answers. Well, we don't either. But if you follow these 10 rules to keeping your kid active, you'll have a great head start.

Rule #1: Don't Rely on Organized Sports

Just because your kid is in T-ball doesn't mean that he's active enough. A new study in the Archives of Pediatrics & Adolescent Medicine found that less than 25 percent of student athletes receive the recommended 60 minutes of daily exercise. Plus, the researchers found that the kids spent about 30 minutes of their practice sessions being completely inactive.

This is so very true. See the habits forming, watching TV for hours at a stretch, the sedentary lifestyle, the lack of stamina when outside playing.

This is something parents need to pay attention to.

How Much Sleep Do Children Need? - NYTimes.com

I have an important question — actually, several related questions — for all parents of school-age children: Do you know how much your children sleep? Do you know how much sleep they really need?

And do you know what their biological clocks are telling them about when to go to sleep and when to wake up?

Although young children are likely to arouse their groggy parents every morning, with no respect for weekends, after puberty the tables turn. Often I hear a familiar lament from parents of adolescents: Every day it’s a struggle to get the kids up and out to school on time.

Elizabeth Glaser Pediatric AIDS Foundation - About Pediatric AIDS

About Pediatric HIV and AIDS

Answers to the most frequently asked questions about pediatric HIV and AIDS.

What is HIV?

The human immunodeficiency virus (HIV) causes acquired immunodeficiency syndrome (AIDS). HIV can be transmitted during sexual intercourse, pregnancy (i.e., from mother to fetus), childbirth, breastfeeding, and other forms of exposure to bodily fluids that carry the virus. When the virus enters the body, it injects itself into vital immune cells called CD4 cells. In the absence of treatment, HIV continues to replicate itself within the body, eventually leading to severe immunodeficiency, chronic illness, and death.

What is AIDS?

AIDS represents an advanced stage of HIV infection. In most cases, a person living with HIV eventually develops AIDS. AIDS is characterized by severely diminished immune system function, where the body is highly vulnerable to infections and cancers that are typically fought off by a healthy immune system. As defined by the U.S. Centers for Disease Control and Prevention, a person has AIDS when HIV has drastically reduced his or her CD4 cell count, or when a person living with HIV is diagnosed with at least one opportunistic infection (i.e., an infection that does not normally occur in someone with a healthy immune system).

A little 8 year old boy wanted to be circumcised when he realized he looked different than dad and his friends. The day after the proceedure he returned to school.

During class, he became uncomfortable and asked for permission to go to the nurse. When he arrived at her office, he hesitated and finally just asked if he could call his mother. Sensing this was personal, the nurse stepped into the hall and closed the door to allow him privacy.

Several minutes later the little boy came out of her office and the nurse noticed his penis was sticking out of his pants. "Where are you going?" she asked. "Back to class," said the boy.

"But you can't go back like that!" explained the nurse. "I have to," stressed the boy. "My mother said that if I could just stick it out until lunchtime, she would come and pick me up."

Children hospitalized longer when parents aren't fluent in English - FierceHealthcare

Children hospitalized for infections were more likely to spend more time in the hospital and were less likely to receive home healthcare referrals if their parents or main caregivers had difficulty speaking English, a new study reports in the Archives of Pediatrics and Adolescent Medicine.

In a nine-year study (between 2000 and 2008), nearly 1,300 children whose parents or caregivers were determined to have limited English proficiency were found to have spent at least 60 percent more time in the hospital when compared to children whose parents/caregivers spoke fluently, said study author Michael Levas, MD, and and his colleagues from Children's Mercy Hospitals and Clinics in Kansas City, Mo.

Overall, most of the parents and caregivers observed by the researchers--nearly 97 percent--were proficient in English. The 3 percent who were not were more likely to be Latino and either uninsured or insured by Medicaid for both comparisons.

Bullying sends kids to nurse for more than injury | Reuters

Both bullies and their victims take more trips to the nurse's office than other students - but not just for the obvious reasons.

In a new study published today in Pediatrics, kids involved in bullying were more likely to see the school nurse for illnesses and non-medical symptoms, in addition to injuries.

The finding means that school nurses could be the front line for spotting cases of bullying and bringing them to a stop.

Enlisting school nurses could be "another way of trying to keep a gauge of what's happening among the students in school," Eric Vernberg of the University of Kansas in Lawrence, the study's lead author, told Reuters Health.

"A lot of times, kids may experience some conflict, some bullying, and not involve the teacher or not tell a parent or not tell anybody really," he said, "particularly for a kid that's embarrassed about the difficulty they're having or afraid that saying things about it is going to make it worse in some way."

Recently, evidence has been piling up that being bullied can take a psychological toll on kids -- both during the bullying and later, when they are adults.

Secondhand smoke may affect kids' mental health | Reuters

Breathing secondhand smoke could increase a child's risk of mental and behavioral disorders, including attention-deficit/hyperactivity disorder (ADHD), suggests a new study.

The study adds to evidence suggesting that kids of mothers who smoked while pregnant may be more likely to have behavioral problems. Secondhand smoke exposure has also been linked to heart and breathing problems in kids.

"It's time for us to begin to prevent children's exposure to (secondhand smoke) if we are serious about preventing these diseases," Dr. Bruce Lanphear, who heads the Cincinnati Children's Environmental Health Center, told Reuters Health.

Summary:  Organ donors are in high demand.  Frequently intended recipients can wait a lifetime for the critical matching organ.  In this case, two nurses obtained a consent from a child's mother.  When the father later expressed his disagreement, the child's corneas had been harvested and it was too late.

The patient was an 11-year-old child that had died soon after an Asthma attack.

"By condition (excluding pregnancy), the five leading causes of hospitalization among children are: bronchitis/asthma, digestive disease, pneumonia and respiratory infection, otitis media and mental disorders."1

The parents were approached for the purpose of donating the child's corneas under the Uniform Anatomical Gift Act.

"What is a Corneal Transplant?

The cornea is a dime-sized clear tissue covering the front of the eye. Light rays pass through the cornea and then through the lens. The lens forms an image on the retina in the back of the eye where the optic nerve is located. Sight is controlled by the optic nerve, the only nerve of vision. This nerve activates the retina to pick up the image in view."2

The mother alone was present at this time when it was first discussed.

"The child's mother maintains that she told the nurse it "did not matter" to her."3

"The Uniform Anatomical Gift Act (UAGA) of 1968 provided for the first time that an individual could donate his or her organs at death to another for medical purposes. It was adopted in all 50 states and the District of Columbia, and was a major step toward solving the organ donation problem. But new medical technologies have now made many transplant operations commonplace, and a shortage of donor organs remains."4

With this, the harvesting center was contacted and the patient's information left.  A consent form was faxed over to the hospital.  Two nurses then spoke to the mother by telephone and signed off on the form that consent had been obtained.

Individual state laws may vary in procedure for who can consent to an anatomical gift.  In Alaska for instance:

"(a) A competent person who is 18 or more years of age may make a gift to take effect upon death of all or a part of the person's body for a purpose specified in AS 13.50.020.

(b) When persons in prior classes are not available at the time of death, and in the absence of actual notice of contrary indications by the decedent or actual notice of opposition by a member of the same or a prior class, any of the following persons, in order of priority listed, may give all or a part of the decedent's body for a purpose specified in AS 13.50.020:

(1) the spouse;
(2) an adult son or daughter;
(3) either parent;
(4) an adult brother or sister;
(5) a guardian of the decedent at the time of death;
(6) any other person authorized or under obligation to dispose of the body.
(c) The persons authorized by (b) of this section may make the gift after or immediately before death."5

The agent for the organ center came and harvested the child's corneas and returned to the center.  He had looked over the consent form obtained, initialed it to verify it had been obtained by phone and was satisfied.

"When the cornea, a thin tissue that covers the front of the eye, becomes cloudy or damaged due to disease, injury or hereditary conditions, the result is vision loss or even blindness.

To clear this window, the damaged cornea is removed surgically and replaced with healthy, transplanted eye tissue-a donated cornea. This transplant operation is successful in more than 90 percent of cases in the US. After a successful transplant, patients have renewed vision or see for the first time."6

Each individual facility must generate protocols on organ procurement and obtaining informed consent:

"Each hospital in the state shall develop procedures for identifying potential donors of gifts, requesting gifts, notifying and coordinating with eye banks, tissue banks, and organ procurement agencies, and assisting in the procurement, removal, storage, and transportation of gifts."7

The child's father would arrive on the scene soon after.  When approached and informed of the harvesting, he was perturbed.  He would not sign the consent after the fact and chose to take legal action against the hospital and the donor center.

In court, summary judgement was entered for the defense:

The parent's appealed.

Questions to be answered:

1. Under the Uniform Anatomical Gift Act (UAGA), is the consent of a parent sufficient to proceed with harvesting of a child's organs.

2. Were the nurses negligent in their explanation of the procedure or in not waiting till the father was available?

The applicable laws stated that as long as a consent from a legally "responsible" and informed party is obtained, no liability or negligence can be assigned.  It was clear from the testimony of the nurses that they spoke to the mother and informed her of what was to happen.

In obtaining organs, time is typically of the essence.  The longer an organ or tissues remains in a body, the less likely it will be useful for the purposes of transplantation.

There was no evidence that the mother was either "coerced" or "rushed" into making a decision.  There was no documentation that she "wished to speak to her husband" before making the decision.  If more time had been needed or another family member needed to be consulted, the mother could have clearly stated this.

On the part of the nurses, a single adult guardian consent only was needed and obtained.

When the agent of the eye bank looked over the informed consent, he as satisfied that it was legitimate.  In harvesting the child's corneas, he acted in good faith and had no reason to believe a proper consent had not been obtained.

The court dismissed the plaintiff's argument that the "good faith" actions of the hospital or eyebank were "subject to interpretation."  The court was satisfied that under the conditions, the actions of the employees were reasonable and within the boundaries of existing law.

This case does point out the need for judgement and clear documentation when a consent for organ donation is obtained.  The nurses were wise to obtain not one but two witness signatures on the consent form.  The agent of the eye bank was wise to make sure the notation was made of a "telephone" consent.

Regardless of these precautions, the nurses, hospital and eyebank were still sued.  Had this lawsuit been initiated after either of the employees involved had left, the hospital may or may not have covered or defended them.

Cuts Leave Patients With Medicaid Cards, but No Specialist to See - NYTimes.com

Eight-year-old Draven Smith was expelled from school last year for disruptive behavior, and he is being expelled again this year. But his mother and his pediatrician cannot find a mental health specialist to treat him because he is on Medicaid, and the program, which provides health coverage for the poor, pays doctors so little that many refuse to take its patients.

Michael Stravato for The New York Times

Dr. Rachel Chatters, right, with Ana Smith, says she begs specialists to see Medicaid patients.

Michael Stravato for The New York Times

Ms. Smith said she has tried for more than a year to find a psychiatrist to treat her son Draven, 8, who is on Medicaid.

The problem is common here and across the country, especially as states, scrambling to balance their budgets, look for cuts in Medicaid, which is one of their biggest expenditures. And it presents the Obama administration with a major challenge, since the new federal health care law relies heavily on Medicaid to cover many people who now lack health insurance.

“Having a Medicaid card in no way assures access to care,” said Dr. James B. Aiken, an emergency physician in New Orleans.

Harmed in the hospital? Should you sue? - CNN.com

Ryan Jeffers finds it hard to believe his daughter, Malyia, went from being a perfectly healthy 2-year-old who loved to dance, sing and entertain to an amputee facing a lifetime of medical care.

"I can't believe something so small turned into something so big," recalls Jeffers, who noticed that his daughter had a fever one Sunday in November.

Malyia's fever continued to climb, and unusual bruises appeared on her cheek; her parents say they rushed her to the emergency room near their Sacramento home. But things did not go as they expected.

The family says that they couldn't get a physician to examine Malyia and that the five-hour wait in the emergency room nearly killed her. Her septic infection worsened as she waited to be seen.

Colds teach the immune system how to fight future viruses

by Michael Gonzalez, MD

Viral upper respiratory illnesses, or common colds, are by far the most frequent illness I see in my office.

It is called the common cold because it is, well, common. I typically can make the diagnosis with a high degree of certainty based on the pattern and the timing of the symptoms. Deciphering cold symptoms and knowing when to worry is what a pediatrician does more often than anything else, and I believe parents can learn how to do this as well. If a parent can get a handle on what a common cold looks like, they can certainly save some co-pays and visits to the doctor. You see, there is no real treatment for the common cold. I know, I know, this is the great failure of modern medicine
